胃癌组织中E-钙粘蛋白表达与浸润转移的关系

Relationship Between the Expression of E-cadherin and the Invasion and Metastasis of Gastric Carcinoma

目的 :探讨E 钙粘蛋白在胃癌中的表达及其与胃癌浸润转移之间的关系。方法 :应用免疫组织化学S P法研究 87例胃癌组织中E 钙粘蛋白的表达。结果 :正常胃组织及其良性病变中 ,E 钙粘蛋白都表达在细胞膜上。 87例胃癌标本中 5 8例 (6 6 7% ,5 8 87)呈异常表达。E 钙粘蛋白异常表达在弥漫型胃癌 (95 8% ,2 3 2 4 )和混合型胃癌 (91 7% ,1 1 1 2 )较肠型胃癌 (4 7 1 % ,2 4 5 1 )显著增高 ,P <0 0 1 ;印戒细胞癌 (85 7% ,1 2 1 4 )和未分化癌 (87 5 % ,1 4 1 6 )较腺癌 (5 6 1 % ,32 5 7)显著增高 ,P <0 0 1。E 钙粘蛋白的表达与肿瘤分化程度 (P <0 0 1 )、浸润范围 (P <0 0 1 )和淋巴结转移 (P <0 0 5 )有显著相关性。结论 :由E 钙粘蛋白介导的细胞间黏附作用在胃癌发生、发展及转移过程中发挥重要作用 ,其在癌组织中表达异常是胃癌重要的恶性生物学特征。正常E

Objectives To evaluate the E cadherin expression pattern in gastric cancer and investigate its relationship to the invasion and metastasis of gastric carcinoma.Methds The expression of E cadherin was evaluated in 87 specimens of gastric cancer by using the immunohistochemistry technique.Results In benign gastric mucosa,E cadherin was expressed uniformly at the cell membrane.Abnormal E cadherin expression was found in 58 (66 7%)cancer pecimens.Aberrant E cadherin expression was found more often in diffuse (95 8%) and mixed type tumors (91 7%) than that in intestinal type tumors(47 1%), P <0 01.A significantly higher proportion of signet ring (85 7%) and undifferentiated tumors(87 5%) showed abnormal E cadherin expression compared with adenocarcinomas with glandular morphology(56 1%), P <0 01.There was a significant correlation between abnormal E cadherin expression and degree of differentiation( P <0 01),the extent of invasion( P <0 01),and the presence of lymph node metastases( P <0 05),respectlvely.Conclusions Intercellular adhesion mediated by the E cadherin plays an important role in morphological changes both in benign and malignant gastric diseases and the abnormality of E cadherin is a significant malignant character of gastric cancer. Loss of normal E cadherin expression may serve as a differentiation marker in gastric cancer.
