Treatments for Patients with Advanced NPC by 120 Hours Infusion of 5-FU and Oral CF Plus CBP

目的 :研究卡铂 (CBP)加 5 -氟尿嘧啶 (5 FU) /醛氢叶酸 (CF)方案治疗晚期鼻咽癌的合适剂量、给药方法及客观疗效和毒副反应。方法 :前瞻性临床研究采用 5 FU 1 2 0h持续输注 ,同时每 4h口服CF 30mg联合CBP方案治疗晚期鼻咽癌 4 1例 ,3～ 4周重复 1次 ,至少用 2个疗程。结果 :4 1例患者共接受 1 5 5个疗程的化疗 ,其中CR 4例 ,PR 2 9例 ,NC 7例 ,PD 1例 ,有效率 80 5 % ;毒副反应主要为口腔炎、恶心和呕吐、骨髓抑制和脱发等 ,其余毒性均为Ⅰ～Ⅱ级 ,腹泻明显少且轻 ,骨髓耐受性好。结论 :5 FU 1 2 0h持续输注并服CF加CBP方案治疗晚期鼻咽癌 ,特别是对复治病变广泛者 ,疗效和安全性高 ,毒副反应小 ,患者可耐受 ,经济实用 。

Objective To explore the proper dosage,administrations,efficacy and side effects of curing advanced nasopharyngeal carcinoma (NPC) by means of carboplatin (CBP) plus 5 FU/CF.Methods The prospective clinical trial was carried out to treat 41 patients with NPC by continuous infusion of 5 FU for 120 hours,at the same time,taking CF30 mg every 4 hours combined with CBP.Treatments were repeated every 3-4 weeks,with no less than 2 courses.Results Forty one patients were given in total 155 courses of chemotherapy treatments.There were 4 cases of CR,29 cases of PR,7cases of NC and 1 case of PD,with the effective rate of 80.5%.The side effects mainly were:stomatitis,nausea,vomiting,arrest of bone marrow and alopecia,while the rest were toxicity stage Ⅰ-Ⅱ.Diarrhea became rare and the tolerance of bone marrow turned good.Conclusion The treatments for patients with advanced NPC by 120 hours continuous infusion of 5 FU and oral CF plus CBP,especially for patients with extensive pathologic change are very efficacious and safe,tolerant and economical,as well as of low side effects.It is definitely worth further research.
