摘要
目的 :探讨血红蛋白浓度与食管癌放射治疗后的生存率及局控率之间的关系。方法 :1993年 2月 - 1998年 1月选择收治食管癌患者 (Ⅰ~Ⅲ期 ,T1-4N0 -1M0 ) 2 39例 ,于放射治疗前根据血红蛋白 (Hb)浓度分 3组 (贫血组、正常组和高血红蛋白组 )。根据放射治疗中血红蛋白浓度的变化分为 2组 (Hb升高组及Hb降低组 )。全部病例均予以根治性放射治疗 ,常规分割 ,对其生存率及局控率予以观察分析。结果 :血红蛋白浓度明显影响食管癌患者的生存率 ;放射治疗前升高血红蛋白量可提高食管癌患者的局控率 ;放射治疗中血红蛋白量的升高及降低对食管癌患者的生存率有影响。结论 :升高血红蛋白量可提高食管癌患者的生存率及肿瘤局部控制率。
Objective To explore the relationship between peripheral blood hemoglobin concentration and the survival and local control rates of patients with esophageal carcinoma after radiotherapy.Methods From February 1993 to January 1998,239 patients with esophageal carcinoma (Ⅰ-Ⅲ stage,T 1-4N 0-1M 0) were selected and treated.The patients were divided into three groups according to the peripheral blood hemoglobin concentration before radiotherapy:anemia group,normal group and high hemoglobin group,and into two groups according to the change in the peripheral blood hemoglobin concentration during radiotherapy as increased and decreased groups.All patients received radical radiation,using conventional fractionation.The survival and local control rates were observed and analyzed.Results The peripheral blood hemoglobin concentration had a significant effect on the survival rates of esophageal carcinoma patients.Increasing the peripheral blood hemoglobin concentration before radiotherapy can improve the local control rates of ESP patients.Its decrease or increase during radiotherapy affected the survival rates of ESP patients.Conclusion Increasing the peripheral blood hemoglobin concentration can improve the survival and local control rates of patients with esophageal carcinoma.
