摘要
目的 探讨经纤支镜毛刷刷脱细胞端粒酶活性与涮洗液癌胚抗原 (CEA)浓度检测对肺癌的诊断价值。方法 采用端粒酶重复序列扩增法和放免法测定分别检测肺癌病变组织 (12 0例 )、非癌性肺疾病 (30例 )纤支毛刷刷脱细胞的端粒酶活性和毛刷涮洗液、血CEA浓度 ,并和病理学及细胞学检查结果进行比较。结果 肺癌病变处纤支毛刷刷脱细胞的端粒酶检出率为 80 % ,非癌性肺疾病纤支镜刷脱细胞的端粒酶检出率 10 % ,二者比较差异显著 (P <0 .0 1) ;肺癌组织部位刷脱细胞端粒酶检出率涮洗液CEA阳性与活检病理阳性率 (73% )及刷检细胞学阳性率 (5 6% )比较 ,差异无显著性 (P >0 .0 5 )。端粒酶阳性与细胞病理类型无关 ,CEA升高以腺癌、小细胞明显。结论 检测纤支镜毛刷刷脱细胞的端粒酶活性。
Objective To explore the diagnostic value of telomerase activity in bronchial brushing cells and CEA in washing fluid and blood from patients with lung cancer for lung cancer.Methods Telomerase activity was measured by PCR-TRAP in cancerous lesions' brushing cells from patients with lung cancer and lesions' brushing cells from patients with noncancerous pulmonary disorders;CEA in bronchial washing fluid and in blood was measured by radioimmunoassay ,in which the specimens were obtained by fiberoptic bronchoscopy.The result detection was compared with the positive rates of pathologic study and bronchial brush cytologic study.Results The positive rate of telomerase activity in lesions of lung cancer(80%) was significantly higher than that of noncancerous pulmonary disorders(10%)(P<0.01).There was no significant difference between the positive rate of telomerase activity expression and the positive rates of pathology and bronchial brush cytology (P > 0.05).Conclusion Examination of telomerase activity in bronchial brushing cells and CEA in washing fluid was of significance in the diagnosis of lung cancer.
