实时三维超声心动图评价冠心病左心房功能的初步研究

Assessment of left atrial function in patients with coronary artery diseases by real-time three-dimensional echocardiography

目的 探讨实时三维超声心动图评价冠心病左心房功能的价值。方法 应用实时三维超声心动图与常规二维超声心动图 ,测量 3 0例正常人与 3 0例冠心病患者收缩末期及舒张末期左心房容积、左心房射血分数。二维超声心动图频谱多普勒测量舒张期二尖瓣口E峰、A峰、E/A值。分别在正常人与冠心病组内 ,比较二维与三维测量结果。结果 ①分别在正常组与冠心病组内 ,二维与三维测量结果比较差异无显著性意义(P >0 .0 5 ) ;②在正常组与冠心病组之间 ,二维超声测量结果、三维超声测量值各自对比差异有显著性意义 (P<0 .0 5 ) ;③三维超声测量的EF值 (LAEF3D)与E/A ,LAEF3D与二维测量的EF值 (LAEF2D)之间具有良好的正相关性 (r =0 .84,r =0 .88)。结论 实时三维超声心动图能通过三维容积测量评价冠心病患者左心房功能 ,具有重要的临床应用价值。

Objective To evaluate left atrial (LA) function in patients with coronary artery diseases (CAD) by real-time three-dimensional echocardiography (RT-3DE).Methods Thirty normal subjects and thirty patients with CAD were examined by two-dimensional echocardiography (2DE) and RT-3DE on their LA end-systolic and end-diastolic volume and LA ejection fraction (LAEF).With 2DE pulse Doppler,the peak flow velocities during early diastole (E),atrial contraction (A),E/A ratio were recorded and calculated.The results by 2DE and RT-3DE of the normal subjects and patients were respectively compared.Results ① In normal subjects or patients with CAD,there was no significant difference between the results measured by RT-3DE and the ones measured by 2DE( P < 0.05 ); ② The results observed by RT-3DE and the ones by 2DE showed significant difference between normal subjects and the patients with CAD( P < 0.05 ); ③ LAEF_ 3D was correlated well with E/A ratio ( r = 0.84 ) and LAEF_ 3D was also correlated well with LAEF_ 2D ( r = 0.88 ).Conclusions The LA function in patients with CAD could be evaluated by RT-3DE,which has great value in clinical practice.
