黏附分子CD44H和血管内皮生长因子协同作用对鼻咽癌复发风险的生物学评估意义(英文)

Significance of biological evaluation for the synergistic action of adhesion molecule CD44H and vascular endothelial growth factor on the recurrent risk of nasopharyngeal carcinoma

摘要 背景:肿瘤复发是残存的肿瘤细胞重新增殖生长为病灶的过程,在此过程中某些基因可能起着重要的调控作用。目的:探讨E-cadherin,CD44H和VEGF基因的表达与鼻咽癌患者复发及康复的关系,探索促进康复的途径。设计:以诊断为依据,标准对照横断面回顾性研究。地点和对象:本研究由汕头市中心医院耳鼻咽喉-头颈外科完成,选择1995年3月~10月的原发性鼻咽癌放疗的62例鼻咽癌患者,年龄17~70岁。方法:免疫组化SP法检测鼻咽癌组织中E-cadherin,CD44H和VEGF的表达强度。以细胞浆内或细胞膜上出现黄色染色为阳性标记,采用半定量方法进行结果判定。主要观察指标:鼻咽癌组织中E-cadherin,CD44H和VEGF的表达强度。结果:在鼻咽癌放疗后复发组中,E-cadherin的表达减弱(阳性表达率为52.4%),CD44H和VEGF的表达增强(阳性表达率分别为85.7%,80.9%),无复发组中三者阳性表达率分别为85.4%,65.9%,73.2%,两组的表达差异均有显著性意义(P<0.05,P<0.01,P<0.01),CD44H与VEGF在鼻咽癌放疗后复发组中的表达有正相关关系(r=0.535,P<0.05)。结论:E-cadherin,CD44H和VEGF的表达均与鼻咽癌放疗后复发的关系密切,CD44H和VEGF存在协同作用,他们可作为评估复发风险的生物学标志,可预测鼻咽癌患者放疗后康复的效果。 BACKGROUND:Neoplastic relapse is a proliferative progression of remained tumor cells.In the progression,some genes may play an important role of regulation. OBJECTIVE:To study the relationship between the gene exprssion of E cadherin,CD44H,VEGF and relapse,rehabilitation of nasopharyngeal carcinoma patients and explore the pathway of rehabilitation. DESIGN:A retrospective study of standardized contrast of cross section was carried out according to diagnosis. SETTING and PARTICIPANTS:This investigation was completed in Department of Otolaryngology Head and Neck Surgery in Shantou Central Hospital.Totally 62 nasopharyngeal carcinoma patients,aged from 17 to 70,receiving radiotherapy from March to October,1995 were chosen. METHODS:Immunohistochemistry SP method was used to detect the expression density of E cadherin,CD44H and VEGF in tissues of nasopharyngeal carcinoma.Yellow staining in cell plasm or on cell membrane was regarded as positive staining. MAIN OUTCOME MEASURE:The expression density of E cadherin,CD44H and VEGF in tissues of nasopharyngeal carcinoma. RESULTS:The expression of E cadherin in nasopharyngeal carcinoma relapse group after receiving radiotherapy decreased(the positive rate of expression was 52.4%);the expression of CD44H and VEGF incresed(the positive rate of expression was 85.7%and 80.9%,respectively).The positive rate of expression of E cadherin,CD44H and VEGF was 85.4%,65.9%and 73.2%in unrelapse group,respectively.The expression difference between the two groups was significant(P< 0.05,P< 0.01,P< 0.01).There was a positive correlation between the expression of CD44H and VEGF in nasopharyngeal carcinoma relapse group after receiving radiotherapy (r= 0.535,P< 0.05). CONCLUSION:The expressions of E cadherin,CD44H and VEGF were closely related to the relapse of nasopharyngeal carcinoma after receiving radiotherapy,and there was synergistic action between CD44H and VEGF.They could be regarded as biological indexes for evaluating the recurrent risk,and they could also be used to predict the rehabilitative effect of nasopharyngeal carcinoma patients after radiotherapy.
