摘要
目的 通过尖牙联合曲 (canineexpansionarchwire ,CEAW )矫治拥挤病例 ,研究矫治前后的牙颌变化情况 ,以丰富拥挤病例非拔牙的矫治方法。方法 选择上牙弓拥挤为Ⅲ度或Ⅲ度以上的 10例患者 (其中男 5例 ,女 5例 ) ,平均年龄 12 .2岁 ,上颌拥挤度平均为 9.12mm。用CEAW进行矫治 ,治疗前后均拍摄头颅定位侧位片 ,并采用配对t检验 ,分析比较矫治前后的X线头影测量结果。结果 ①坐标系中 ,上颌第一磨牙牙冠远中点向后的移动、上齿槽缘点和上中切牙切缘点向前的移动均具有统计学意义 ( P <0 .0 5 )。②相对于腭平面 ,上颌第一磨牙牙冠远中点向后的移动、上齿槽缘点和上中切牙切缘点向前的移动均具有统计学意义 (P <0 .0 1)。③同时发现上颌第一磨牙明显后倾 (P <0 .0 1) ,而上颌切牙没有明显唇倾。结论 CEAW通过后倾磨牙、推磨牙向后可以迅速而有效地解除拥挤 。
Objective To evaluate the dentomaxillary change of the crowding cases treated with non-extraction and followed by canine expansion archwire (abbr.CEAW).Methods 10 patients (5 males,5 females) with no less than 8mm crowding in the upper arch were treated by CEAW.The pre- and post-treatment cephalometric films were compared.All data were statistically analyzed by t-test using the SPSS 10.01 software.Results With reference to the coordinates and PP plane,the retrusion of the molars and protrusion of the incisors were both significant(P< 0.05 ).Maxillary first molar was inclined distally(P< 0.01 ).No statistical significance was found on incisor inclination.Conclusion CEAW can eliminate the crowding effectively by pushing and inclining molars distally,at the same time the incisors move forward.
