通冠胶囊对冠心病介入术后血脂含量和凝血功能的影响

Effect of Tongguan Capsule on Blood Lipid Content and Blood Coagulation After Percutaneous Coronary Intervention

【目的】探讨通冠胶囊在治疗冠心病介入术后患者中的降血脂和改善血瘀证候作用。【方法】将127例行冠心病介入术后患者分为中西医结合组(57例)和西医组(70例),分析两组患者的一般临床资料、手术参数,测定血脂和平均红细胞体积(mean corDuscular volume,MCV)、红细胞分配宽度(red blood cells volurne distribution width,RDW-CV)、平均血小板体积(mean platelet volume,MPV)以及血小板分配宽度(platelet distribution width,PDW)。【结果】两组治疗前的相关参数均无显著性差异(P>0.05),即治疗前的基线水平一致。中西医结合组的血中总胆固醇(TC)在术后30 d为(4.55±0.81)mmol·L^(-1),较治疗前的(5.39±0.96)mmol·L^(-1)为低(P<0.001),亦较西医组术后30 d的(5.41±0.95)mmol·L^(-1)为低(P<0.01),而低密度脂蛋白(LDL)和甘油三酯(TG)在术后7d和30d虽有逐渐下降的趋势,但未见显著性差异(P>0.05)。两组均有不同程度的降低MCV的作用(与治疗前比较P<0.01或P<0.001),且中西医结合组对MCV、PDW的改善作用也优于西医组(P<0.05或P<0.001)。【结论】通冠胶囊对冠心病介入术后患者有较好的降血脂作用和改善血瘀证候的作用。

[Objective] To investigate the effect of Tongguan Capsule on blood lipid content and blood coagulation in patients after percutaneous coronary intervention (PCI). [Methods] One hundred twenty- seven patients with PCI were randomized to two groups: group A (n=57, treated with traditional Chinese and western medicine) and group B (n=70, treated with western medicine). The clinical characteristics, PCI parameters and blood lipid content were evaluated. Also, the mean corpuscular volume (MCV), red blood cells volume distribution width (RDW-CV), mean platelet volume (MPV) and platelet distribution width (PDW) were detected. [Results] The general conditions in the two groups before treatment revealed the same baseline. Thirty days after PCI, the level of total cholesterol (TC) was (4.55±0.81) mmol·L^(-1) in group A, which was lower than (5.39±0.96) mmol·L^(-1) before treatment in group A (P<0.001) and (5.41±0.95) mmol·L^(-1) in group B after treatment (P<0.01). Serum levels of low- density lipoprotein (LDL) and triglyceride (TG) tended to decrease but the difference was insignificant before and after treatment in group A (P>0.05). The levels of MCV, MPV and PDW in the two groups were lower than those before treatment in varying degrees (P<0.01 or P<0.001), and the effects of group A in improving MCV and PDW level were better than those of group B (P<0.05 or P<0.001). [Conclusion] Tongguan Capsule exerts a better effect in decreasing blood lipid level and in improving blood stasis in patients after PCI.
