Semiarthroplasty in the treatment of senile intertrochanteric fracture

目的 探讨人工股骨头置换治疗高龄转子间骨折的可行性、可靠性。方法 采用人工股骨头置换治疗高龄转子间骨折 2 15例 ,均采用骨水泥假体固定 ,术前全组患者合并有内科疾患的 190例 ,占 88% ,经内科治疗后手术。结果 围手术期死亡 5例 ,切口感染 9例 ,术后早期脱位 4例 (2例再手术 )。术后 2年以内因内科疾患死亡和失访者 6 0例。对 15 0例患者进行随访 ,资料完整者 10 2例 ,优良率 95 %。结论 人工股骨头置换治疗高龄转子间骨折应严格掌握适应证和技术操作要点 。

Objective To investigate the feasibility and reliability of semiarthroplasty in the treatment of senile intertrochanteric fracture.Methods 215 cases were treated with semiarthoplasty,of which 190 cases (88%) were associated with other diseases, and were undergone relative treatment before operation.Results 5 cases died perioperatively,9 got wound infection and 4 got early joint dislocation,of which 2 cases received revision.60 cases were missed to follow up because of death or other causes in 2 years postoperatively.150 cases were followed up.The excellent and good rate was 95%. Conclusion It could be a regular treatment for senile intertrochateric fracture with semiarthroplasty,but strict indication and skillful technique are mandatory,and it is the linchpin to treat other diseases perioperatively.
