自制微泵局部抗生素注入治疗急性化脓性骨髓炎 被引量:5

摘要 目的 观察自制微泵局部抗生素注入治疗化脓性骨髓炎的临床效果。方法 将敏感抗生素溶液放入自制微泵中通过与之相连的硬膜外麻醉导管以 4~ 6ml/h(最大流量 34ml/h)的速度持续注入化脓性骨髓炎病灶 6~ 10周。结果  18例患者全身中毒症状和局部炎症在 3~ 7d得到有效控制。随访 18例 ,随访时间最长 2年 4个月 ,最短 6个月 ,平均 1年 3个月 ,17例愈合 ,1例复发。结论 利用自制微泵局部抗生素注入治疗化脓性骨髓炎可有效保证病灶中抗生素浓度 ,能提高化脓性骨髓炎的治愈率 。 Objective To observe clinical effect of the treatment of acute pyogenic osteomyelitis with local antibiotic lavage by selfmade minipump. Methods 18 Cases of acute pyogenic osteomyelitis were enrolled. One end of peridural catheter was connected with osteomyelitis focus and the other end with the self-made minipump. Sensitive antibiotic solution was perfused into the focus by the minipump at speed of 4~6 ml/h(34 ml/h in maximum) for 6~10 weeks. Results Systemic toxic symptoms and local inflammatory reaction of 18 cases were controlled within 3~7 days. All of the cases were followed up from 6 to 28 months, 15 months on average. 17cases were cured. 1 case was recurred. Conclusion Local antibiotic lavage by selfmade minipump can effectively maintain antibiotic dosage in osteomyelitis focus. Therefore,the healing rate of pyogenic osteomyelitis can be improved. The treatment is expected to be a new therapy for acute osteomyelitis.
