后路椎间植骨椎弓根钉系统内固定治疗腰椎滑脱症 被引量:5

摘要 目的 报道后路椎体间植骨融合椎弓根钉系统内固定术治疗腰椎滑脱的疗效。方法 应用后路椎体间自体髂骨植骨融合椎弓根钉系统内固定治疗腰椎滑脱 36例 ,Ⅰ度滑脱 18例 ,Ⅱ度滑脱 15例 ,Ⅲ度滑脱3例 ;峡部裂型 30例 ,退变性 5例 ,外伤性 1例。结果 随访 6个月~ 2年 ,术后 6个月植骨处均骨性愈合 ,无内固定物松动、脱出或断裂现象。临床疗效综合评价 :优 14例 ,良 2 0例 ,可 2例。结论 后路腰椎椎体间植骨融合加椎弓根钉系统复位固定治疗腰椎滑脱可以取得良好复位、坚强内固定、彻底减压。 Objective To report the experience on lumbar spondylolisthesis treated by posterior lumbar intervertebrae fusion(PLIF) and pedicle screw fixation. Methods 36 cases with isthmic (30 cases), traumatic (1 case) and degenerative (5 cases) spondylolisthesis were treated with posterior lumbar interbody fusion with autograft of ilium fixation by and pedicle screw system. Among them, 18 cases were classified as degreeⅠ, 15 as degree Ⅱ and 3 as degree Ⅲ. Results All cases were followed up for 6~24 months. All cases got bony union in 6 months postoperatively. No looseness, protrusion and breakage of internal fixator were found. The results were excellent in 14, good in 20 and fair in 2. Conclusion PLIF and pedicle screw system fixation for spondylolisthesis can provide good reduction, rigid fixation, complete decompression as well as high rate fusion.
