AO外固定架治疗胫腓骨开放性粉碎性骨折

Treatment of open tibia-fibula shatter fracture with AO external fixator

目的探讨AO外固定架治疗胫腓骨开放性粉碎性骨折的优越性及并发症的防治。方法利用AO外固定架治疗胫腓骨开放性粉碎性骨折63例,按Gustilo分型:Ⅰ型11例,Ⅱ型29例,ⅢA型13例,ⅢB型8例,ⅢC型2例。结果随诊4个月～4年,平均19个月,4例发生浅表感染,感染率为6.33%,63例均达骨性愈合。骨折愈合时间为4～12个月,平均7.1个月。临床疗效满意。结论应用AO外固定架治疗胫腓骨开放性粉碎性骨折手术创伤小,操作方便,符合外固定原则,可早期进行功能锻炼,有利于骨折愈合和肢体功能恢复。

Objective To study the principle, feasibility, advantages, and prevention of complications in the treatment of open tibia fibula comminuted fracture with AO external fixators. Methods 63 cases of open tibia fibula comminuted fracture, including 11 type I, 29 type Ⅱ , 13 type IIIA, 8 type IIIB, and 2 type IIIC, according to Gustilo classification, were reduced and fixed with AO external fixators. Results All patients were followed up for 4 48months, with the average time being 19 months. 4 patients suffered superficial infection,and the infection rate was 6.33% . All patients had bony union within 4 12 months, with the average time of bone union being 7.1months. The outcome was satisfactory. Conclusion The advantages of AO external fixators in treatment of open tibia fibula comminuted fracture are less traumatic, easy to use, devoid of joint stiffness and muscular atrophy, and beneficial to fracture healing and early functional recovery.
