视网膜脱离术后再脱离的原因分析

The causes of redetachment of postoperative retinal detachment

目的 分析视网膜脱离 (简称网脱 )术后再脱离的原因 ,推荐较佳的手术方式。方法 总结 2 12例经巩膜外行视网膜脱离手术后再脱离的病例 ,归纳出再脱离的原因以及再手术方式的选择。结果 2 12例患者中有14 2只眼行玻璃体切割、硅油 (或膨胀气体 )填充 ;其余均行巩膜外手术。出院时 186例视网膜完全复贴 ,18例视网膜局限性浅脱离 ,4例无效 ,4例放弃治疗。结论 仔细搜寻裂孔 ;准确、定量封闭裂孔 ;正确处理 PVR、缓解牵引是保证手术成功的关键。玻璃体切割对特殊类型的网脱起着不可替代的作用 ,但并不适合所有类型的网脱病例。相反对玻璃体切割手术应正确掌握手术指征 ,因为玻璃体是不可再生的组织 。

Objective To analysis the causes of redetachment of postoperative retinal detachment.Methods 211 cases of redetachment of postoperation by outside sclera were investigated,looking for causes,elected the quomodo of reoperation.Results 142 cases underwent pars plana vitrectomy,filling in silicon oil(or gas).The other cases were performed outside sclera.The retina of 186 cases were reattached (reposition),local detachment in 18 cases,no effect in 4 cases,quit operation in 4 cases.Conclusion It is key of operation successful to search holes carefully,nicety and fixed quantity blockade holes,exactly treatment PVR.Vitrectomy is first-rate method for especial types retinal detachment(RD),but it disagree with the whole the RD.
