白内障超声乳化人工晶状体植入联合小梁切除术

Phacoemulsification and foldable intraocular lens(IOL)implantation combined with trabeculectomy

目的 评价白内障超声乳化摘出、折叠式人工晶状体植入联合隧道内小梁切除术 (三联手术 )治疗青光眼合并白内障的疗效。方法 采用隧道式巩膜瓣切口对 2 4例 (2 8眼 )青光眼合并白内障施行三联手术。结果 术后随访末期 ,最终矫正视力为 0 1～ 1 0 ,其中 0 1～ 0 5者 15眼 (53 6% ) ,0 6～ 0 8者 10眼 (3 5 7% ) ,1 0者 3眼 (10 7% )。术后眼压全部降至正常范围 ,平均眼压 (14 3± 6 5)mmHg(1mmHg =0 13 3kPa)。术后有角膜水肿 8眼 (2 8 6% ) ,浅前房 2眼 (7 1% ) ,后发性白内障 2眼 (7 1% )。结论 白内障超声乳化吸出折叠人工晶状体植入联合隧道内小梁切除术治疗青光眼合并白内障是一种安全。

Objective To evaluate the effect of phacoemulsification,foldable intraocular lens(IOL)impl antation combined with trabeculectomy(triple procedure)in patients with glaucoma and cataract.MethodsTriple procedure was performed through a 3 5mm incisioin by means of scleral tunnel flap on 28 eyes of 24 patients with gla ucoma and cataract.ResultsAfter the follow-up period,the best c orrected visual acuities were 0.1-1.0,of which 15 were 0.1-0.5(53.6%),10 were 0. 6-0.8(35.7%),3 were 1.0(10.7%).Postoperatively,the intraocular pressure(IOP) of all the eyes decreased to normal(mean,14 3±6.5mmHg),8 eyes had comeal edema(28 .57%),2 eyes had shallow anterior chamber(7.15%),2 eyes developed after cataract (7 14%).ConclusionsThe triple procedure of phacoemulsification ,f oldable intraocular lens implantation combined with trabeculectomy is safe and e ffective for treating coexisting glaucoma and cataract,it can reduce intraocular pressure,improve visual acuity rapidly with few postoperative complications.
