中后颅窝型三叉神经鞘瘤的手术治疗

Surgical Treatment of MP Trigeminal Neurinoma

目的 :提高中后颅窝型三叉神经鞘瘤的切除程度和术后疗效。方法 :对收治 2 5例这类病例进行回顾性分析。结果 :15例手术行颞底 经天幕入路 ,余 10例行枕下乙状窦后等其他入路。肿瘤全切 2 2例 ,次全切除 2例 ,大部切除 1例。结论 :中后颅窝型三叉神经鞘瘤显微手术全切是其最佳方法 ,颞底 经天幕入路是一良好手术途径 ,其术野显露充分 ,易于达到肿瘤全切 ,微创 。

Aim:To raise the removal degree and to improve the postoperative result of MP (middle and posterior fossae) trigeminal neurinoma.Methods:Aretrospective review of 25 patients with MP trigeminal neurinoma treated in our department was undertaken. Results:Subtemporal-transtentorial approach was used for 15 patients. Suboccipital retrosigmoid and other appraochs were used for 10 patients. Total tumor removal was achieved in 22 patients, subtotal in 2, and largely partial in 1.Conclusion:The best treatment for MP trigeminal neurinoma is complete microsurgical removal of the tumor through subtemporal-transtentorial approach.
