慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭无创通气的应用

Application of Noninvasive Bi-level Positive Pressure Ventilation in Patients with COPD and Type Ⅱ Respiratory Failure

目的 讨论无创通气在慢性阻塞性肺疾病并Ⅱ型呼吸衰竭中的临床效果。 方法 对 4 0例慢性阻塞性肺疾病并呼吸衰竭患者 ,采用BiPAP呼吸机治疗 ,与常规组对照比较患者呼吸频率及心率血气分析。结果 发现双水平气道正压 (BiPAP)呼吸机可迅速提高患者的氧分压 ,降低二氧化碳分压 ,纠正呼吸衰竭 ,降低呼吸频率及心率 ,改善临床症状及体征。 结论 应用BiPAP呼吸机 ,鼻 /面罩式机械通气可迅速提高患者的氧分压 ,降低二氧化碳分压 ,纠正呼吸衰竭 ,降低呼吸频率及心率。

Objective Investigation of the effect of noninvasive bi-level positive pressure ventilation in patients with COPD and degree Ⅱ type respiratory failure.Methods 40 patients with COPD and degree Ⅱ type respiratory failure were randomized into ventilation treatment group and drug treatment group.20 patients were treated with noninvasive ventilation.Results Noninvasive ventilation could increase PaO 2 within 4 hours in the two groups(P<0.05),there was no significant change of PaCO 2 within 4 hours between the two groups(P>0.05).Conclusion Noninvasive ventilation was effective for patients with COPD and degree Ⅱ type respiratory failure.It may reduce endotracheal intubation and be used in clinical practice.
