高压氧治疗2760例脑器质性病变的疗效评价

Evaluation on the therapy effect of hyperbaric oxygen of 2 760 patients with the organic encephalopathy

目的 研究高压氧 (HBO)治疗一氧化碳 (CO)中毒、颅脑外伤和脑血管病的疗效。方法 选择CO中毒 90 0例 ,颅脑外伤 72 6例 ,脑血管病 1 1 34例 ,采用不同的治疗方案 ,评价治疗效果。结果 轻、中度CO中毒治愈率 1 0 0 % ,重度治愈显效率 94 .30 % ,迟发性脑病 71 .4 3% ;颅脑外伤治愈显效率 :轻型 98.5 1 % ,中、重型 87.71 % ,持续性植物状态 78.95 % ;椎 基底动脉供血不足基本治愈显效率 90 .2 2 % ,脑出血、脑梗塞为 78.5 3%。HBO治疗后血粘度明显下降 (P <0 .0 5 ,0 .0 0 1 ) ;LPO下降 ,SOD上升 (P<0 .0 0 1 ) ;脑动脉的脉动指数 (PI)下降 (P <0 .0 5 ,0 .0 1 )。结论 HBO联合药物治疗CO中毒、颅脑外伤和脑血管病有显著的疗效。

Objective To study the therapeutic effect of hyperbaric oxygen(HBO) on CO intoxication,craniocerebral damage and cerebrovascular disease.Methods Nine hundreds patients with CO intoxication,726 with craniocerebral damage and 1 134 with cerebrovascular disease were selected.The patients were treated with several different methods and the therapeutic effects were evaluated.Result The curative rate in mild and moderate CO intoxication was 100%,the marked effectve rates(MER) in severe and delayed brain disease was 94.30%,71.43% respectively;MER in mild,moderate and severe and prolonged vegetation status of craniocerebral damage was 98.51%,87.71%,78.95% respectively;MER in insufficient blood of vertebral basic artery,brain haemorrhage and infarction was 90.22%,78.53% respectively.The blood viscosity had a dramatic reduction after HBO( P <0.05,0.001);the plasma lipid peroxidation(LPO) decreased and the RBC superoxide dismutase(SOD) elevated ( P <0.001).The pulsating index(PI) of the brain artery decreased( P <0.05,0.01).Conclusion HBO combined with medicine has a significant therapeutic effect on CO intoxication,craniocerebral damage and cerebrovascular disease.
