20例肺错构瘤的外科治疗

Surgical treatment of 20 pulmonary hamartomas

目的 对 1 985～ 2 0 0 2年 1 7年间经手术治疗 ,术后病理证实的 2 0例肺错构瘤进行分析研究 ,探讨肺错构瘤的临床特点 ,诊断和外科治疗方法。方法 2 0例肺错构瘤病人 ,年龄 2 5～ 6 3岁 ,平均 4 7.6岁 ,其中男性 1 4例 ,占 70 %。全部病人均采用手术治疗 ,其中肿瘤摘除 4例 ,肺楔形切除 1 1例 ,肺段切除 2例 ,肺叶切除 2例 ,1例因肿瘤巨大而行全肺切除术。结果 2 0例病人术后均经病理证实为肺错构瘤 ,绝大多数为软骨型肺错构瘤 ,术后恢复顺利。随访 1 9例 ,失访 1例。 1例因肿瘤复发 3次行手术治疗 ,本组未发现肺错构瘤伴发恶性病变。结论 肺错构瘤有复发可能 ,应对患有肺错构瘤的病人进行全面评估和经常的随访。

Objective To recognize the clinic characteristics,methods of diagnosis and surgical treatment of 20 pulmonary hamartomas in 17 years.Methods Twenty cases of pulmonary hamartoma were analyzed,aged 25 to 63 years with a mean age of 47.6 years,including 14 men,4 were operated on for pulmonary hamartoma enucleations,11 wedge resections,2 segmentectomies,2 lobectomies,and one patient underwent pneumonectomy for huge tumor.Results All of 20 patients were proved with pathologic diagnosis of pulmonary hamartoma and the chondromoid type was the most common.The popcorn calcification was found in 3 cases.In 19 cases having the follow up of 2 years,a patient was operated again because of recurrence.We did not find the associated malignancies.Conclusions Because of the possible recurrence,patients with pulmonary hamartoma should be submitted to a complete evaluation and regular follow up.
