抑郁性失眠与心理生理性失眠多导睡眠图特征比较 被引量:8

摘要 目的:探讨抑郁性失眠与心理生理性失眠多导睡眠图参数变化特征。 方法:对27例快动眼潜伏期明确缩短的抑郁性失眠(depression or dysthymia insomnia,DDI)和39例心理生理性失眠(psychophysiologic insomnia,PPI)患者的多导睡眠图结果进行回顾性统计分析,并进行两组间睡眠多导图各参数的比较。 结果:①快动跟测量值:快动眼潜伏期(min):DDI组(65.6±11.9)较PPI组(156.7±67.0)明显缩短(t=8.295,P=0.000);快动眼周期数(次):DDI组(4.2±1.1)较PPI组(3.3±1.0)明显增多(f=3.110.P=0.003)。但快动眼时间(min)两组差异无显著性意义(t=1.862.JD=0.067)。②睡眠结构:非快动眼时间占总睡眠时间百分数(%)DDI组较PPI明显减少(t=3.145,P=0.003);但其组分S_1%,S_2%,S_(3-4)%两组分别比较却无明显差别。快动眼时间占TST百分数(快动眼%)DDI组较PPI组明显增多(t=3.164,P=0.002)。但在所有66例失眠患者中S_1%:男21.6±0.09,女15.7±0.08,有显著差异(t=3.145.P=0.006)。③睡眠进程及连续性:在睡眠潜伏期,醒起时间,觉醒时间,醒觉次数,总睡眠时间,觉睡比,睡眠效率,两组分别比较差异均无显著性意义(P>0.05)。 结论:抑郁性失眠的快动眼测量值及睡眠结构较心理生理性失眠患者具有特征性的变化,非快动眼减少而快动眼增加;但? AIM: To explore the characteristics of changes of the polysomnogram(PSG) between depressive insomnia and psychophysiologic insomnia. METHODS: Twenty-seven cases of depression or dysthymia insomnia(DDI) with definitely shortened rapid eye movement (REM) latency and 39 cases of psychophysiologic insomnia (PPI) were selected. A retrospective statistical analysis of the results of PSG was conducted, and the indices of PSG were compared between the two groups. RESULTS: (1) REM measured value: REM latency(min) in the DDI group (65. 6 ±11. 9) was obviously shorter than that in the PPI group(156. 7± 67.0) (t = 8. 295, p = 0. 000); REM cycle(time) in the DDI group(4. 2 ± 1.1) increased significantly compared with the PPI group(3. 3 ± 1. 0) (t = 3. 110, P = 0. 003); while there was no difference in REM time(min) between the two groups( t = 1. 862, P = 0. 067) .(2) Sleep structure: the percentage of non-REM time in total bedtime in the DDI group reduced obviously ( t = 3. 145, P = 0. 003), while there was no significant difference between the two groups in S1%, S2%, S3 -4%, the percentage of REM time in TSTin the DDIgroup increased more obviously than in the PPI group (t = 3. 164, P = 0. 002), but there was significant difference in S1% between males(21.6±0. 09) and females(15. 7 ±0. 08) (t = 3. 145, P = 0.006) . (3) Sleep process and continuity: there were no significant differences in sleep latent period, time from wake-up to get-up, awakening time, number of awakening, total sleep time, awakening ratio, sleep efficiency and sleep maintenance rate between the two groups ( P > 0. 05). CONCLUSION: Compared with PPI, REM value and sleep structure have characteristic changes, non-REM reduces while REM increases; while there is no difference in sleep process and continuity between them; besides, stage I superficial sleep is more in males than in females.
