大豆异黄酮等复合抗氧化剂对2型糖尿病患者血脂代谢的影响 被引量:9

摘要 目的:探讨大豆异黄酮与维生素C、维生素E的协同作用对2型糖尿病患者血脂代谢的影响。 方法:将120例符合研究条件的2型糖尿病患者随机分为糖尿病非干预组、低剂量组、中间剂量组和高剂量组,与30例正常人对照。分别检测口服葡萄糖耐量试验(OGTY)时0,1,2,4,6h血液中三酰甘油、胆固醇、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low densitylipoprotein cholesterol,LDL-C)、载脂蛋白A1(apolipoprotein A1,ApoA1)、载脂蛋白B(apohpoprotein B,ApoB)的动态变化。 结果:OGTT后,正常对照组各项血脂指标与空腹比较均无显著性变化。4个糖尿病组中非干预组餐1,2 h后的三酰甘油显著高于0 h(F=5.05,4.52,P<0.05);餐后6h总胆固醇显著高于0,1h(F=3.86,3.91,P<0.05);餐后4,6hLDL-C显著高于0 h(F=4.34,6.28,P<0.05);餐后6hApoB显著高于正常对照组(F=4.31,P<0.05)和高剂量组(F=3.99,P=0.046)。低剂量组餐后1,2 h的三酰甘油显著高于Oh(F=4.51,4.46,P<0.05);餐后6h LDL-C显著高于0 h(F=5.38,P<0.05);餐后6 hApoB显著高于正常对照组(F=4.29,P<0.05)和高剂量组(F=3.87,P<0.05)。而中剂量组和高剂量组则呈剂量依赖性表现出降低三酰甘油、胆固醇、LDL-C,ApoB和升高HDL-C,ApoA1。 AIM: To observe the co-effect of isoflavone, vitamin C (VC) and vitamin E (VE) on the blood lipid metabolism in type 2 diabetes mellitus. METHODS: Totally 120 type 2 diabetic patients who met with the research criteria were divided into non-intervention group, low, middle and high dose groups and 30 normal subjects were taken as the control group. The levels of triacylglycerol(TG), cholesterol, high density lipoprotein cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C), apolipoprotein Al(ApoAl) and apolipoprotein B(ApoB) were measured when 1,2,4 and 6 h after oral glucode tolerance test( OGTT). RESULTS: The indexes after OGTT were not significantly different from fasting ones in the control group. In the non-intervention group, the 1, 2 h TG levels were markedly higherthan that of 0h(F = 5. 05,4.52, P <0.05); 6h postprandial cholesterol markedly higher than those of 0, 1 h( F = 3. 86,, 3.91..P <0.05); 4, 6 hLDL-C markedly higherthan that of 0 h (F=4. 34, 6. 28, P <0. 05); 6 h ApoB was significantly higher than those in the control group(F = 4. 31, P <0. 05) and the high dose group( F = 3. 99, P = 0.046). TG, cholesterol, LDL-C and ApoB decreased, while HDL-C, ApoAl and ApoAl/ApoB increased in the middle and high dose groups, and they were dose dependent: 2 h ApoAl /ApoB in the middle dose group was significantly higher than that of 0 h( F = 3. 99, P < 0. 05); 6 h LDL-C was significantly lower than that in the non-intervention group(F =4. 99, P<0. 05) . In the high dose group, 6 h cholesterol was significantly lower than that of 0 h (F = 6. 17, P < 0. 05); 4, 6 h LDL-C was significantly lower than that in the non-intervention group(F = 4. 68, 5. 83, P < 0. 05). The 2 h ApoAl /ApoB in the control group, non-intervention group, low, middle and high dose groups were 1. 87 ±0. 45,1. 63±0. 35,1. 52 ±0. 33,1. 66 ±0. 32,1. 89 ±0. 32, re- spectively, significantly higher than the fasting ones (1.86 ±0.58, 1. 62 ± 0. 41,1. 48 ±0. 32,1. 45 ±0. 41,1. 66 ±0. 43)(F = 4. 86, P <0. 05). CONCLUSION: In middle and high dose groups, the mixture of isoflavone, VC and VE, can obviously improve the metabolic disorder of TG, cholesterol, LDL-C and ApoB caused by high glucose peroxidative stress in patient with type 2 diabetes, and the mixture can also increase the HDL-C,ApoAl and ApoAl/ ApoB.
