自制“灌洗装置”在大肠癌术中的应用研究

An applied research on lavage device in the operation of large intestine

目的探讨术中结肠排污(减压)和灌洗的理想方式。方法应用自制并获得国家专利(专利号:ZL93220204.7)的全封闭式肠道术中排污和灌洗装置(以下简称装置)行52例(A组)结、直肠癌术中排污和灌洗,并与传统的灌洗方法(B组)对照观察。结果应用本装置对52例结、直肠癌行术中排污与灌洗后,肠管Ⅰ期吻合40例,腹壁人工肛门12例。术中全无粪水污染腹腔,无闻臭气。整个灌洗过程顺畅,灌洗速度快,肠道清洁度高。术后无吻合口瘘、出血和狭窄等并发症。腹壁切口全部I期愈合。结论本装置突破了国内外传统的开放式或相对封闭式的术中结肠灌洗模式,开启了完全封闭式的术中结肠灌洗的新进程。值得临床推广应用。

Objective To search an ideal method of decompression and lavage in the operation of large intestine. Methods The full-sealed off devise for decom pression and lavage of colonic procedure was used in 52 cases with colon and rec tal cancer(group A),and compared with the traditional method(group B). Among gro up A,primary anastomosis of intestine and artificial anus of abdominal wall-wer e done in 40 cases and 12 cases respectively.Results There were no pollution of abdominal cavity and bad smell during operation.All the course of lavage was flu ent,fast and intestinal cleaning was satisfactory.No anastomotic leakage,bleedin g and stenosis were found after operation.The incisional frist intention was obt ained in all cases. Conclusion This apparatus breaches traditional domestic and abroad open method or relative closed method on the irrigation of colon during o peration.A new progress is started on the full-sealed colonic lavage during ope ration.It is worthily recommended in the clinical application.
