中西医结合治疗应激性溃疡40例疗效观察

Therapeutic Effect of Integrative TCM and WM in Treating Stress Ulcer,a Report of 40 Cases

目的 :观察中西医结合疗法治疗应激性溃疡的临床疗效。方法 :将 6 6例应激性溃疡随机分为 2组。治疗组 4 0例采用中西医结合疗法 ,对照组 2 6例仅用西药治疗。结果 :治疗组 1天内止血 2 6例 ,1～ 2天止血 10例 ,3～ 4天止血 4例 ,止血率为 10 0 .0 % (95 % CI=91.2 %～ 10 0 .0 % ) ;对照组 1天内止血 8例 ,1～ 2天止血 10例 ,3～ 4天止血 3例 ,5～ 7天止血 2例 ,死亡 3例 ,止血率为 88.5 % (95 % CI=6 9.8%～ 97.5 % ) ,死亡率为 11.5 % (95 % CI=2 .5 %～ 30 .2 % ) ;两组止血综合疗效比较 ,有明显差异 (u=2 .75 0 2 ,P=0 .0 0 78)。治疗组止血时间为 1.90± 1.35天 ,对照组为 2 .91± 1.86天 ,两组比较 ,差异显著 (t=- 7.314 3,P<0 .0 0 1)。结论 :中西医结合治疗应激性溃疡优于西药治疗。可缩短止血时间 ,减少死亡率 ,其收益为 NNT=9(95 % CI=4 .5 9～ 81.97)。

Objective:To observe the therapeutic effect of integrative TCM and WM in treating stress ulcer.Method:40 patients in the treatment group were treated with integrative TCM and WM,26 patients in the control group were treated only with WM.Result:In the treatment group,26 patients stopped bleeding within 1 day,10 patients stopped bleeding after 1 day and within 2 days,4 patients stopped bleeding after 3 days and within 4 days.The haemostatic haemostatic rate was 100.0%(95%CI=91.2%～100.0%).While in the control group,8 patients stopped bleeding within 1 day,10 patients stopped bleeding after 1 day and within 2 days,3 patients stopped bleeding after 3 days and within 4 days,2 patients stopped bleeding after 5 days and within 7 days,and 3 patients died.The haemostatic rate was 88.5%(95%CI=69.8%～97.5%).The fatality rate was 11.5%(95%CI=2.5%～30.2%).The difference of total therapeutic effects between the two groups was notable (u=2.7502,P=0.0078).The average time for haemostasis in the treatment group and in the control group were 1.90±1.35 days and 2.91±1.86 days respectively.The difference was notable (t= 7.3143,P<0.001).Conclusion:Integrative TCM and WM had better therapeutic effect than the control group in treating stress ulcer.It could shorten the haemostatic time,decrease the fatality rate.NNT=9(95%CI=4.59～81.97).
