摘要
目的 探讨乳腺良性病变常规增强MRI表现与微小血管的关系。方法 5 6例经手术病理证实的乳腺良性病变 ,术前行MR平扫及常规增强扫描 ,并测定MR信号强度增强率 ( SI)。术后标本行免疫组织化学染色测定病灶内微小血管密度及分布情况 ,并分析与相应病灶 SI之间的关系。结果 微血管在乳腺纤维腺瘤、腺病及导管内乳头状瘤的中心和边缘区域分布无显著性差异 (P >0 .0 5 ) ,常规增强扫描 ,相应区域MRI对比剂的分布也无显著性差异 (P >0 .0 5 ) ,5 6例乳腺良性病变中心和边缘区域的微血管密度 (MVD)与 SI之间存在正相关关系 (P <0 .0 1)。而小血管多分布于乳腺纤维腺瘤、乳腺病和导管内乳头状瘤的边缘区域 ,与 SI之间无显著相关性 (P >0 .0 5 )。结论 乳腺良性病变常规增强MRI表现与微血管的数量和分布有关。
Objective To investigate the relationship between conventional contrast enhanced MRI features and microvessel of benign breast lesions. Methods Fifty-six patients with surgically and histopathologically verified benign breast lesions underwent plain and conventional contrast-enhanced MR scanning before operation. The signal intensity enhancement rate (SI) of conventional contrast-enhanced MRI were calculated. The mastectomy specimens of the patients with benign breast lesions were stained with immunohistochemistry for detecting the density and distribution of micro and little vessel. The relationship between the micro and little vessel and SI was analyzed. Results No significant difference of the distribution of micro vessel was observed between the center and periphery areas of breast fibroadenoma,mastopathy and intraductal papilloma. And no significant difference of the distribution of MR contrast agent of conventional contrast-enhanced MRI was observed in the corresponding areas. There existed significantly positive correlation between the microvessel density (MVD) and SI of center and periphery areas of 56 patients with benign breast lesions. The micro vessel distributed more in the periphery areas of breast fibroadenoma, mastopathy and intraductal papilloma. There were no significant correlations between the micro vessel and SI of center and periphery areas. Conclusion Conventional contrast-enhanced MRI features of benign breast lesions are correlated with the density and distribution of microvessel.
