慢性心力衰竭患者左心功能与血浆内皮素-1水平的关系

Relationship between left cardiac function and endothelins-1 levels in patients with chronic heart failure

目的:探讨慢性心力衰竭患者左心功能与血浆内皮素-1(endothelins-1,ET-1)水平的关系。方法:对70例慢性心衰住院患者,按纽约心脏学会分级,Ⅱ级35例,Ⅲ级16例,Ⅳ级19例;对照组20例(健康体检者),用特异性放免法测定血浆ET-1水平,并用超声心动图参数评价左心功能。结果:①随心功能恶化,血浆ET-1水平呈上升趋势,各级心功间差异均有显著性(t=2.573～3.786;P<0.05)。②经抗心力衰竭药物综合治疗后,ET-1水平降低(t=3.574,P<0.01)。③ET-1水平与左心室射血分数呈负相关(r=-0.77,P<0.05);与左室舒张末内径呈正相关(r=0.62,P<0.05)。结论:慢性心力衰竭患者血浆内皮素-1与心力衰竭程度相关,可作为血液指标用于监测心衰程度。

AIM:To explore the relationship of left cardiac function with endothelins 1(ET 1) levels in patients with chronic heart failure(CHF). METHODS:Seventy hospitalization patients with CHF were classified with New York Heart Association(NYHA) into grade Ⅱ(n=35),grade Ⅲ(n=16) and grade Ⅳ(n=19).Twenty healthy persons were in the control group.The ET 1 levels were determined with the specific radio immunoassay, and the left cardiac function was evaluated with echocardiogram parameters. RESULTS:①The levels of ET 1 increased with the deterioration of heart function,there were significant differences in heart function among different grades(t=2.573-3.786,P< 0.05).②The levels of ET 1 were obviously reduced after comprehensive drug treatment of anti CHF(t=3.574,P< 0.01).③The levels of ET 1 were negatively correlated with the left ventricle ejection fraction (r=-0.77,P< 0.05) and positively correlated with left ventricle end diastolic diameter(r=0.62,P< 0.05). CIn patients with chronic heart failure,ET 1 has a close relationship with the levels of heart failure,and can work as a blood director to monitor the extent of the levels of heart failure.
