近视眼角膜中央厚度与眼压和屈光度的关系 被引量:8

摘要 目的 :探讨近视眼角膜中央厚度 (centralcornealthickness,CCT)与眼压 (IOP)、屈光度之间关系。方法 :10 0例193只眼行角膜中央厚度、眼压、屈光度、眼轴检查 ,并对所得数据行方差分析、直线回归分析。根据角膜中央厚度分四组 :1组 4 4 5 μm <CCT <5 0 0 μm ,2 9只眼 ;2组 5 0 0 μm≤CCT <5 5 0 μm ,91只眼 ;3组 5 5 0 μm≤CCT <6 0 0 μm ,6 6只眼 ;4组CCT≥ 6 0 0 μm ,7只眼 ;计算相对应的眼压值 ,并行方差分析。根据近视屈光度不同分为 4组即低度组 ( <- 3.0 0D) ,6 6只眼 ;中度组 ( - 3.0 0D≤屈光度 <- 6 .0 0D) ,79只眼 ;高度组( - 6 .0 0D≤屈光度 <- 9.0 0D) ,2 2只眼 ;超高度组 (≥ - 9.0 0D) ,2 6只眼。分别计算其相对应的眼压和角膜中央厚度值 ,对获得的数据行方差分析、直线回归分析。结果 :角膜中央厚度与眼压呈正相关关系 ,回归方程为Y =- 0 .30 710 + 0 .0 338X(角膜中央厚度为X ,眼压为Y) ,t=6 .96 6 4 ,P =0 .0 0 ,r=0 .4 5 11。根据角膜中央厚度分组 ,4组的眼压均值分别为1组 ( 13.79± 2 .76 )mmHg、2组 ( 14 .4 9± 2 .5 5 )mmHg、3组 ( 16 .0± 1.99)mmHg和 4组 ( 2 2 .5 2± 2 .94 )mmHg ,方差分析F =2 9.6 9,P =0 .0 0 0 ,组间比较除 1组与 2组间差异无显著性 (q =1.92 。 Objective:To investigate the correlation of central corneal thickness(CCT) and intraocular pressure(IOP) and refraction in myopic eyes.Methods:The CCT and eye axial length were measured by ultrasonic,IOP by noncontact tonometer,refraction by keratometer in 193 eyes of 100 myopias. The results were tested by analysis of variance and linear regression analysis. The 193 eyes were divided into four groups depending on CCT,the group one(CCT<500 μm) in 29 eyes,group two(500 μm≤CCT<550 μm) 91 eyes,group three(550 μm≤CCT<600 μm) 66 eyes and group four(CCT≥600 μm) 7 eyes. The data were divided into four groups depending on refractive diopter,mild group(<-3.00 D) in 66 eyes,moderate group(-3.00~-6.00 D) 79 eyes,high group(-6.00 D≤refractive diopter<-9.00 D) 22 eyes and supra high group(≥-9.00 D) 26 eyes. All data were tested by analysis of variance.Results:There was positive correlation between CCT and IOP in myopia. To take CCT as X and IOP as Y,then a linear regression formula Y=-0.30710+0.0388X was concluded(t=6.9664,P=0.000,r=0.4511). The mean value of IOP were(13.79±2.76)mmHg,(14.49±2.55)mmHg,(16.0±1.99)mmHg and (22.52±2.94)mmHg for groups one,two,three and four,respectively. There was significant difference between groups except between groups 1 and 2(q=1.92,P>0.05) depending on the result of analysis of variance(F=29.69,P=0.000). There was no correlation between CCT and refraction in myopia(t=1.3758,P=0.1705>0.05,r=0.0993). The mean value of IOP were(15.1±2.84)mmHg,(14.85±2.49)mmHg,(14.82±2.18)mmHg and (15.28±2.10)mmHg for mild,moderate,high and supra high groups,respectively. There was no significant difference between groups depending on the result of analysis of variance(F =0.2673,P=0.8489). The mean CCT were(543.5±31.62)μm,(530.22±36.78)μm,(531.23±32.73)μm and (524.44±26.04)μm for mild,moderate,high and supra high groups,respectively,respectively. There was significant difference between groups depending on the result of analysis of variance(F=2.89,P=0.0367).Conclusion:There is a significant positive correlation between IOP and CCT in myopic eyes,especially the CCT being more than 550 μm and less than 450 μm,the IOP being increased with CCT. There was no correlation between IOP and refractive diopter.
