萘丁美酮不良反应发生的危险因素分析

摘要 目的 :探索萘丁美酮不良反应发生的有关危险因素和计算不良反应发生概率的方程。方法 :经过分层整群抽样 ,对 14 5例服用萘丁美酮的患者进行了回顾性流行病学调查。采用R×C卡方或Wilcoxon秩和检验进行单因素分析 ;采用Logistic逐步回归进行多因素分析。结果 :单因素分析结果显示“疾病年限”、“家庭收入”、“非甾体抗炎药不良反应家族史”、“对自己的健康负责程度”、“服用咖啡的频率”、“情绪变化对生活和工作的负面影响”、“感觉生活有意义 ,充满挑战”、“感觉事业成功”与不良反应的发生有关。进一步用logistic逐步回归分析显示5个因子“不良反应家族史”、“情绪变化对生活和工作的负面影响”、“家庭收入”、“对自己的健康负责程度”、“感觉生活有意义 ,充满挑战”与不良反应的发生相关。同时得到了估算不良反应发生概率的线性方程。结论 :该方程可指导医师在处方萘丁美酮时对该药的利益 危险均衡作出判断。 Objective: To screen out some risk factors of the adverse drug reactions (ADRs) to Nabumetone and then establish a model to predict the incidence of ADR. Method: By stratified cluster sampling, a retrospective epidemiological survey was conducted on 145 patients who had used Nabumetone in the treatment. R*C Chi square test and Wilcoxon test were adopted for a mono factorial analysis while the logistic stepwise regression was conducted for a multi factorial analysis.Result: The Results of the mono factorial analysis indicated that the 'duration of disease', 'family income', 'family history of ADR caused by NSAID', 'degree of personal care', 'frequency of coffee consumption', 'adverse effect of emotional problem on daily work and life', 'interest in every challenge' and 'feeling of self fulfillment' were related to the occurrence of ADR. The logistic stepwise regression analysis further showed that the 'family history of ADR caused by NSAID', 'adverse effect of emotional problem on daily work and life', 'family income', 'degree of personal care' and 'interest in every challenge' were closely related to the occurrence of ADR. A linear equation to forecast the incidence of ADR was established according to the results.Conclusion: The linear equation would provide some guidance for clinicians in prescribing Nabumetone.
