糖尿病相关的情绪改变及其与病情控制的关系

Diabetes-Related Emotional Disturbance and Its Relationship with Glycemic Control

目的 :了解现患糖尿病患者与糖尿病相关的情绪改变及其与病情控制的关系。方法 :采用糖尿病相关的问题量表 (PAID)以问卷形式调查 76名现患糖尿病患者与糖尿病相关的问题共 2 0道 ,同时将患者按最近 1- 2月的糖化血红蛋白 (HbA1c)水平分为病情控制良好组 (A组 ,HbA1c <7% )、病情控制一般组 (B组 ,9.0 % >HbA1c≤7 0 % )及病情控制欠佳组 (C组 ,HbA1c≥ 9.0 % )。比较三组患者PAID评分情况 ,分析PAID评分与HbA1c水平的关系。结果 :现患糖尿病患者存在不同程度的情绪障碍 ,主要表现为抑郁、焦虑、愤怒等。三组患者PAID评分存在显著性差异 (P <0 .0 5 ) ,直线相关分析显示PAID评分与HbA1c显著相关 ,表现为HbA1c水平越高PAID评分越高(P <0 .0 5 )。结论 :糖尿病患者的病情控制与其情绪障碍密切相关 ,PAID评分有助于临床上更全面地评估糖尿病患者病情控制情况。

Objective:To make clear the relationship between diabetes related emotional disturbance and the degree of glycemic control in diabetic patients. Methods:A total of 76 patients with diabetes were divided into three groups according their most recent glycohemoglobin A1c(HbA1c)levels(group A:HbA1c<7.0%;group B:9.0%>HbA1c>=7.0%;group C:HbA1c>=9.0%).Each patient was accepted a questionnaire of the Problem Areas in Diabetes Scale (PAID) to obtain his or her PAID scores.We analyzed the difference of the scores among the three groups and the relationship between PAID scores and glycemic control.Results: In these patients with diabetes,some have had different degree of emotional disturbance,such as depression,anxiety and angry.One way analysis showed that the PAID scores were significantly different in three groups(group C>group B>group A, P< 0.05),and PAID scores were significantly related to the levels of HbA1c ( P< 0.05),suggesting the higher the PAID scores,the worse the glycemic control.Conclusions: Emotional disturbance in diabetic patients is closely related to their glycemic control.PAID scores may help to evaluate diabetic progression comprehensivly in clinical practice.
