尿毒清颗粒剂治疗118例慢性肾衰竭的疗效观察

Observation of the Curative Efficiency of Uremic Clearance Granules in 118 Patients with Chronic Renal Failure

目的 :探讨中药尿毒清颗粒剂与西医结合治疗慢性肾衰竭 (CRF)患者对改善肾功能和延缓CRF进展的疗效。方法 :将 2 36例CRF患者随机分成两组。对照组 118例仅予西医治疗 ,治疗组 118例在西医治疗的基础上服用尿毒清颗粒剂 2 5～ 4 0 g/d ,疗程 2个月。比较两组临床疗效、副作用及对CRF进展的影响。 结果 :治疗组总有效率为 6 0 .17% ,明显高于对照组的 2 2 .88% (P <0 .0 1) ;各种症状均有明显改善 ,改善率 70 %以上 ,明显优于对照组 (P <0 .0 5 ) ;治疗后血肌酐 (Scr)和BUN均显著降低 (P <0 .0 5 ) ,RBC明显升高 (P <0 .0 1)。而且治疗组中 ,Scr≤ 4 4 2 μmol/L者疗效明显优于Scr≥ 4 5 1μmol/L者 (P <0 .0 1) ,在 18例Scr≥ 70 7μmol/L患者中 ,仅 2例有效 ,与对照组比较无统计学差异 (P >0 .0 5 )。 1年内治疗组中有 5 / 18的患者进入终末期 (ESRD) ,显著低于对照组的 13/ 2 0 (P <0 .0 5 )。治疗过程未见明显毒副作用。结论 :中药尿毒清颗粒剂与西医结合治疗CRF 。

Objective:To investigate the curative efficiency of integration of Chinese medicine Uremic Clearance Granules(UCG) and western medicine(WM) on chronic renal failure(CRF) in improvement for renal function and retard for progression of CRF. Methods:236 patients with CRF were randomly divided into control group(n=118) which was treated with WM alone and treated group(n=118) which was added with oral UCG 25 to 40 grams everyday for two months. The curative efficiency , which was assessed by the changes of serum creatinine(Scr),BUN,and clinical signs and symptoms, was compared between two groups together with side effect and progression of CRF. Results:The overall effective rate(60.17%) in treated group was significantly higher than that(22.88%) in control group(P<0.01). All the clinical signs and symptoms in treated group had been ameliorated with the rates more than 70%, which were significantly higher than that in control group respectively(P<0.05). Both Scr and BUN were significantly decreased (P<0.01), and the level of red blood cells (RBC) was significantly increased (P<0.05) after the follow-up of two months in treated group,but the changes of these indices in control group were not significant. Furthermore,in treated group,the curative efficiency of the patients with Scr≤442 μmol/L was significantly superior to that with Scr≥451 μmol/L(P<0.01),but only 2 of 18 cases with Scr≥ 707 μmol/L showed effective,that there was no significance compared with control group (P>0.05). During the follow-up of one year, 5 of 18 cases in treated group entered the end-stage renal disease (ESRD), while 13 of 20 patients in control group did. A significant difference existed (P<0.05). Eventually, there was no obvious side effect during the follow-up. Conclusion:The curative efficiency of integration of Chinese medicine UCG and WM on CRF is better than that of WM alone. UCG can improve renal function and retard the progression of CRF.
