低氧诱导因子-1α表达与胰腺癌增殖和凋亡的关系 被引量:5

EXPRESSION OF HYPOXIA-INDUCIBLE FACTOR-1 ALPHA AND APOPTOSIS AND PROLIFERATION IN PANCREATIC CANCER

摘要 ①目的 研究胰腺癌组织中低氧诱导因子 1α(HIF 1α)的表达与胰腺癌增殖和凋亡的关系。②方法应用免疫组织化学方法 ,检测 4 7例胰腺癌和 1 0例正常胰腺组织中HIF 1α、增殖细胞核抗原 (PCNA)及Bcl 2蛋白的表达。③结果  4 7例胰腺癌组织HIF 1α表达阳性率为 5 5 .3% ,1 0例正常胰腺组织均呈阴性表达。HIF 1α阳性表达率在有周围脏器浸润的胰腺癌组织中为 6 6 .7% ,无浸润者为 35 .3% ,二者间有显著性差异 (χ2 =4 .32 ,P <0 .0 5 ) ;伴淋巴结和远隔脏器转移的胰腺癌组织HIF 1α阳性表达率为 72 .0 % ,高于无转移者的 36 .4 % ,其差异有显著意义 (χ2 =6 .0 1 ,P <0 .0 1 )。HIF 1α阳性表达率与胰腺癌的大小、组织学分级和 1年生存率无关 (χ2 =2 .0 7~ 2 .5 2 ,P >0 .0 5 )。在 4 7例胰腺癌组织中高增殖活性 2 8例、低增殖活性 1 9例。 1 0例正常胰腺的腺胞均呈低增殖状态。胰腺癌细胞增殖程度与胰腺癌淋巴结或远处脏器转移及病人术后生存时间均密切相关 (χ2 =5 .98、6 .1 9,P <0 .0 5 ) ;而与胰腺癌的大小、分化程度和浸润无关 (χ2 =2 .0 1~ 3.74 ,P >0 .0 5 )。 1 0例正常胰腺癌组织Bcl 2蛋白均呈阳性表达 ,4 7例胰腺癌组织中Bcl 2蛋白的阳性表达率为 2 9.8%。Bcl 2蛋白表达与胰腺癌的大小、 Objective To study the expression of hypoxia inducible factor-1 alpha (HIF-1α) and its relationship with proliferation and apoptosis in human pancreatic cancer. Methods The immunohistochemical PicTure TM method was used to detect the expressions of HIF-1, Bcl-2 and PCNA in 47 cases of pancreatic cancer and 10 cases of normal pancreas tissues, respectively. Results In 47 cases of pancreatic cancer, the positive rate of HIF-1α was 55.3%, but zero in the 10 cases of normal pancreatic tissues. Significant difference existed between HIF-1α expression and infiltration and metastasis in pancreatic cancer (χ 2=4.32,6.01, P<0.05,0.01). The positive rate of HIF-1α had no relation with the size, differentiation and 1-year survival rate of the patients ( χ 2= 2.07- 2.52, P>0.05). The extent of proliferation of pancreatic carcinomas was closely correlated with metastasis and 1-year survival rate (χ 2=5.98, 6.19, P<0.05), but not with the size, histological grade and invasion (χ 2=2.01-3.74, P> 0.05 ). The positive rate of Bcl-2 was 29.8% and 100% in 47 cases of the carcinomas and 10 cases of normal pancreatic tissue, respectively, and it was not related with the clinical pathological features of the cancer (χ 2=0.65-2.11, P>0.05). The expression of HIF-1α was positively associated with the PCNA level (r=0.586, χ 2=15.15, P<0.01), but not with the expression of Bcl-2 ( r= -0.211 , χ 2=2.094, P>0.05) in pancreatic cancer. Conclusion Over-expression of HIF-1α is significantly associated with the proliferation, invasion and metastasis, but not with the apoptosis of pancreatic cancer. The expression level of PCNA is an important index for judging the metastasis and prognosis of the disease.
