应用CPM和康复疗法预防及治疗烧伤后膝关节僵硬及腘窝挛缩畸形的对照研究

A control study of treatment and prevention the rigor of knee joint and the contracture deformity of popliteal fossa after intensity burn by CPM and rehabilitation therapy

目的研究关节持续被动活动(continuouspassivemotion,CPM)和康复疗法预防及治疗深度烧伤后膝关节僵硬及窝挛缩畸形的效果。方法34例下肢较深度烧伤(包括窝周围皮肤烧伤)患者随机分为CPM组(19例)和对照组(15例),对照组应用常规康复疗法,CPM组应用常规康复疗法+持续被动活动器(CPM)进行康复治疗。结果分别比较康复前后膝关节伸屈主动ROM和被动ROM,CPM组和对照组均有显著性差异(P<0.01);CPM组与对照组康复后的膝关节主动ROM和被动ROM相比较有显著性差异(P<0.05)。结论应用常规康复疗法和常规康复疗法+CPM均可有效地预防和治疗膝关节僵硬及窝瘢痕挛缩畸形,但常规康复疗法+CPM的疗效更好。

Objective:To explored the effect of continuous passive motion and rehabilitation to treat and prevent the rigor of knee joint and the contracture deformity of popliteal fossa caused by scarring after intensive burning. Method: 34 patients with intensive burn on lower limb (containing the around skin of popliteal fossa ) were divided randomly into CPM group(n=19) and control group(n=15). The CPM group received both routine rehabilitation and continuous passive motion treatment, and the control group only underwent routine rehabilitation treatment.Result:There was a marked improvement both in the CPM group and the control group of active and passive range of motion of knee joint after treatment(P<0.01). The CPM group was more effective to improve the active range of motion and passive range of motion of the knee joint(P<0.05). Conclusion:Application of routine rehabilitation treatment and both rehabilitation treatment and CPM is able to effectively prevent and treat the rigor of knee joint and the deformity of popliteal fossa caused by scarring contraction. But the remedial effect by both rehabilitation and CPM is more excellent compared with that of routine rehabilitation treatment only.Author′s address Industrial Injury Rehabilitation Hospital,Guangzhou,510970
