正常妊娠晚期母婴乙型肝炎表面抗体浓度相关性研究

Correlation of the concentration of HBsAb between normal late pregnant women and their newborns

目的 定量研究正常妊娠晚期母婴乙型肝炎表面抗体 (HBsAb)浓度相关性。方法 1 5 2例乙型肝炎表面抗原阴性产妇 ,分娩前 2 4h抽静脉血 ;其新生儿收集脐血。用酶联免疫吸附 (ELISA)法检测母血HBsAb ;用时间分辨免疫荧光分析 (TRF)法测HBsAb定性阳性的母血及其相应脐血的HBsAb浓度。结果 ELISA法检测母血HBsAb阳性率为 2 8.95 % (4 4 / 1 5 2 ) ;TRF法测母血HBsAb浓度 >1 0mU/mL者 4 1例 (>1 0mU/mL为阳性 ) ,与ELISA法相符率为 93.1 8% (4 1 / 4 4 ) ,测母血HBsAb阳性对应脐血的HBsAb浓度 >1 0mU/mL者 4 2例。 4 4例母血HBsAb阳性组中 ,其相应脐血有 6 8.1 8% (30 / 4 4 )HBsAb浓度高于母血。结论 正常妊娠晚期HBsAb可通过胎盘主动转运至胎儿体内 ,脐血中HBsAb浓度与母血呈显著正相关 。

Objective To analyse the correlation of the concentration of HBsAb between normal late pregnant women and their newborns. Methods One hundred and fifty two pregnant women with negative HBsAg were drawn blood 24 hours before delivery, and their newborns' umbilical blood were also collected. HBsAb in mothers' blood was detected by ELISA method; the concentrations of mothers' confirmed HBsAb positive blood and blood of their respective newborns were detected by time resolved fluoroimmunoassay (TRF) method. Results The HBsAb positive rate of mothers' blood was 28.95% (44/152); 41 of which had HBsAb concentration of more than 10 mU/mL (HBsAb concentration of blood more than 10 mU/mL by TRF detection can be defined as HBsAb positive), the coincident rate with ELISA was 93.18% (41/44), the concentrations of HBsAb in 42 newborns' umbilical blood were more than 10 mU/mL, 68.18% (30/44)of newborns' blood concentration was higher than that of mothers. Conclusion HBsAb can be transported from normal late pregnant women's placenta to the fetus body, there is positive correlation between the concentration of HBsAb in mothers' and fetus' blood.
