血管内超声消融术治疗难治性下肢动脉硬化闭塞症31例报告

Application of intravascular ultrasound ablation in treating refractory lower extremity artery atherosclerotic occlusion:a report of 31 cases

目的 总结应用Acolysis超声消融仪治疗难治性下肢动脉硬化闭塞症的治疗体会。 方法 2 0 0 2年 2月至 2 0 0 3年 10月对 31例难治性下肢动脉硬化闭塞症病人采取小切口暴露股动脉并切开 ,引入超声导管 ,DSA监视下消融 ,至血管再通。并结合其他腔内治疗和旁路转流术治疗。结果 全部病例术后踝弘指数 (ABI)均较术前明显改善。结论 超声消融术与其他血管腔内成形术、旁路转流术综合治疗难治性动脉硬化闭塞症 ,近期疗效满意 ,为临床提供了一条新的治疗途径。

ObjectiveTo summarize the experience of application of intravascular ultrasound ablation in treating refractory lower extremity artery atherosclerotic occlusion. MethodsThe data of 31 patients with refractory lower extremity artery atherosclerotic occlusion treated from February 2002 to October 2003 were analysed. Femoral ateries were exposed through a small incision and excided, and the ultrasound catheter were inserted, then ablation performed until recanalization was obtained. This technique was combined with transluminal angioplasty or /and bypass. ResultsPostoperatively all patients got significant improvement in ankle brachial index(ABI). ConclusionShort-term outcome of intravascular ultrasound ablation in combination with transluminal angioplasty or /and bypass in treating refractory lower extremity artery atherosclerotic occlusion is satisfactory, which offers a new therapeutic choice for these patients.
