胰岛素瘤的诊断和治疗(附128例报告)

Diagnosis and therapy of insulinoma:a report of 128 cases

目的 总结胰岛素瘤的外科治疗经验。方法 回顾性分析 1983～ 2 0 0 2年收治的 12 8例胰岛素瘤的临床资料。结果 肿瘤局部摘除术适用于 75 8%的病人 ;胰瘘是胰岛素瘤手术后最常见的并发症。结论 胰岛素瘤的手术方式应该根据病变的性质、部位、数量等因素决定 ;必要的术前定位检查有助于术中选择合理的手术方式。

Objective To summarize the surgical therapeutic experience of insulinoma in order to improve the operating results. Methods The clinical data of 128 patients of insulinoma or nesidioblastosis from 1983 to 2002 were analyzed retrospectively. Results Enucleation was suitable for 75.8 percent of patients;Pancreatic fistula was the most frequent complication.Conclusion The operation should be based on such properties as the nature,the quantity and the spot of lesions.Necessary preoperative locating examinations are beneficial to the choice of operations.
