眼内异物摘出联合白内障摘出人工晶状体植入

Extraction of intraocular foreign body combining with cataract extraction and IOL implantation

目的 评价眼内异物伴外伤性白内障行异物摘出联合白内障摘出人工晶状体植入术的临床疗效。方法 对 2 5例 (2 5眼 )眼内异物伴外伤性白内障依异物的位置不同而采用 3种术式 :采用玻璃体切除 (9眼 )或磁铁吸出玻璃体异物 (10眼 ) ,联合晶状体超声乳化及折叠人工晶状体植入 ;或经角膜缘切口摘出晶状体内异物联合囊外摘出PMMA人工晶状体后房植入 (6眼 )。结果 随访 3月 ,术后视力≥ 0 5 15眼 ,0 1～ 0 48眼 ,0 0 82眼。术后视力低的主要原因是眼铁质沉着症。术后视力与角膜伤口的大小部位有关 ,与视网膜异物与黄斑区的距离有关。术后无眼内炎或视网膜脱离的发生。结论 准确判定预后。

Objective To investigate the clinical effect of extraction of intraocular foreign body combining with cataract extraction and IOL implantation.Methods 25 eyes suffered from intraocular foreign body and traumatic cataract were divided into 3 groups according to the different position of intraocular foreign body .9 eyes were treated by vitrectomy combining with Phaco and foldable IOL implantation.10 eyes were treated by magnet extraction of vitreous foreign body combining with Phaco and foldable IOL implantation.6 eyes were treated by ECCE and lenticular foreign body extraction combining with PC_IOL(PMMA) implatation.Results 3 months later,the visual acuity (VA) of all the 25 eyes got rise,>0 5 (15 eyes),0 1～0 4(8 eyes),0 08(2 eyes).The main cause of low sight was siderosis.The influence of VA included the position and length of corneal wound as well as the distance between the foreign body and macula.There was no complication such as endophthalmitis and retinal detachment.Conclusion Extraction of intraocular foreign body combining with IOL implantation is practicable.It can shorten the treatment cycle as well as there is no obvious direct correlation complication.
