慢性肾功能衰竭原发病的流行病学研究

An Epidemiological Study on Primary Diseases of CRF

目的 探讨慢性肾功能衰竭患者原发病的发病率及中医辨证特点。方法 回顾性调查慢性肾功能衰竭患者的各种原发病的发病率 ,病例分别来自中国大陆、港、澳、台地区及东南亚和欧美地区 ,共 976例。结果 慢性肾功能衰竭患者的原发病中 ,慢性肾炎最为常见 ,其它依次为高血压肾病、糖尿病肾病和慢性肾盂肾炎 ;而中国港、澳、台地区及东南亚和欧美地区的糖尿病肾病的发病率高于高血压肾病 ;慢性肾盂肾炎和系统性红斑狼疮性肾炎女性较男性常见 ,而高血压肾病则男性较女性多见 ;血液透析的患者中 ,原发疾病居首位的是糖尿病肾病 ;依据中医辨证论治原则 ,全部病例均具有“虚、瘀”表现 ,而另有“挟湿”、“挟逆”或同时“挟湿、挟逆”的不同。结论 慢性肾炎是引起慢性肾功能衰竭首位原发病 ,且慢性肾功能衰竭原发病的分布有地区和性别差异。通过本次流行病学分析 ,从中、西医两个方面 ,为肾脏病更深层次的研究奠定了基础。

Objective To explore the incidence of the primary diseases and the syndrome differentiation of CRF.Method A retrospective study wais used to explore the incidence of the primary diseases of CRF. Total 976 subjects came from the mainland, Hong-Kong and other areas.Result It showed that CGN was the most prevalent primary disease, the incidence of DN in Hong-Kong was higher than in mainland, the incidence of chronic pyelonephritis and LN in female was higher than in male, DN was the most frequent primary disease by HD, asthenia and stasis occurred in all patients with damp pathogen or (and) subjugating pulse. This study established a solid foundation for the further research of kidney disease. Conclusion CGN was the most Prevalent one among the primary diseases of CRF. There were differences in area and gender distribution in the primary diseases of CRF.
