MRI对自发性肾脏出血的评价

MRI Evaluation of Spontaneous Renal Hemorrhage

目的 探讨MRI对自发性肾脏出血的诊断价值。资料与方法 回顾性分析 2 1例不同病因所致自发性肾脏出血的MRI资料 ,主要观察MRI对出血灶的显示、出血责任病变的检出及出血的病因诊断。结果 MRI清晰地显示了 2 1例共 2 5个出血灶的部位、形态、范围 ,甚至肾实质的破裂口 ,而不同时期出血的信号强度具特征性 ,据此可大致判定出血时间。MRI对出血责任病变的检出率为 6 0 % ,对出血灶肿瘤性病因诊断准确率为 80 % ,对非肿瘤性病因诊断准确率为 5 4 .5 %。结论 MRI能敏感地发现自发性肾脏出血 ,准确估计出血部位、出血量并可判定出血时间 ,具有较高的出血责任病变检出率和病因诊断准确性 。

Objective To evaluate MRI in the clinical diagnosis of spontaneous renal hemorrhage.Materials and Methods MRI findings in 21 patients with renal hemorrhage due to different renal disorders were retrospectively analyzed. The observation was focused on the demonstration of the hemorrhage, the responsible disease and the cause of the hemorrhage.Results (1) MRI well displayed the site, shape and extent of all the 25 hemorrhagic lesions, and the renal parenchymal laceration in 21 cases. From the characteristics of signal intensity at different stages, the ages of the hemorrhage could be roughly judged. (2) The detecting rate of the responsible lesions for MRI was 60%. (3) The diagnosis accuracy of hemorrhagic etiology was 80% for tumors and was 54.5% for non-tumors. Conclusion MRI is very sensitive in detecting spontaneous renal hemorrhage. It can precisely determine the bleeding site and volume, estimate the age of hemorrhage, find out the responsible lesion and make a correct etiologic diagnosis. Therefor, MRI is of great value in the diagnosis of the disease.
