摘要
目的 :探讨膀胱移行细胞癌 (Transitionalcellcarcinomaofthebladder ,BTCC)组织中血管内皮生长因子 (Vas cularendothelialgrowthfactor,VEGF)的表达与微血管生成的关系及临床意义。方法 :应用免疫组织化学二步法检测 10例正常膀胱粘膜、5 0例BTCC组织中VEGF的表达和微血管密度 (Microvasculardensigy ,MVD) ,并与临床病理资料进行分析。结果 :① 5 0例BTCC组织VEGF阳性表达率为 72 % ,MVD为 4 .4~ 13.4个 /高倍视野 ,平均值8.94± 2 .0 8,10例正常膀胱粘膜VEGF的表达均为阴性 ;MVD0 .6~ 1.6个 /高倍视野 ,平均值 1.2 0± 0 .35 ;②浸润型肿瘤组较表浅型肿瘤组MVD高 (P <0 .0 1) ,G2 和G3 级肿瘤组较G1级肿瘤组MVD高 (P <0 .0 1) ,高MVD组较低MVD组复发率高 (P <0 .0 5 ) ;③浸润型肿瘤组较表浅型肿瘤组VEGF阳性表达率高 (P <0 .0 1) ,G2 和 (或 )G3级肿瘤组较G1级肿瘤组VEGF阳性表达率高 (P <0 .0 1) ,VEGF阳性表达组BTCC复发率高 (P <0 .0 1) ;④VEGF阳性表达组MVD高 (P <0 .0 1)。结论 :①BTCC中VGEF的表达以及MVD与正常膀胱粘膜相比具有明显的特征性 ;②VEGF的表达以及MVD与BTCC分期、细胞的分级、复发相关 ,VEGF、MVD均为较理想的预测膀胱癌复发的标志物 ;③VEGF表达与MVD相关 ,VEGF能促进BTCC?
Objective: To investigate the relationship between the expression of vascular endothelial growth factor(VEGF) and angiogenesis and the clinical significances in transitional cell carcinoma of the bladder(BTCC).Methods:Immunohistochemistry(two step method) was used to detect the expression of VEGF and FⅧ RAg in 10 tissues specimens of normal mucosa of bladder and 50 tissues specimens of primary BTCC.The results was analysed with the clinic pathological characters of BTCC.Results:①In 50 tissues of BTCC,the positive expression rate of VEGF was 72%, the average microvascular density(MVD)value was 8.94±2.08(range 4.4 to 13.4).While in 10 tissues of normal mucosa of bladder,the expression of VEGF was negative and the average MVD value was 1.20±0.35(range 0.6 to 1.6);②The average MVD Value was higher in the group of invasive stage( P <0.01)and high grade(G3 and G2)( P <0.01),The recurrent rate was higher in the group of high MVD( P < 0.05 );③The positive expression rate of VEGF was higher in the group of invasive stage( P < 0.01 )and high grade(G3 and or G2)( P <0.01),The recurrent rate was higher in the group that the expression of VEGF was positive( P <0.01).The average(MVD)value was higher in the group that the expression of VEGF was positive( P <0.01).Conclusion:①The expression of VEGF and MVD have significant differences between the tissues of normal mucosa of the bladder and the tissues of the BTCC;②The expression of VEGF and MVD have prognostic value in BTCC;③The expression of VEGF has correlation to MVD and VEGF has microvascular angiogenesis effect in BTCC.
