雾化吸入液渗透压对痰液外排的影响 被引量:27

摘要 目的 探讨痰液外排的雾化液最适宜的渗透压条件。方法 以慢性阻塞性肺疾病 (COPD)患者为观察对象 ,雾化吸入渗透压梯度增高的雾化液 ,分别收集雾化吸入前后 1h的痰液 ,对照观察痰液总量、干 /湿比重、黏度及中性粒细胞膜结合弹力酶表达水平。结果 蒸馏水吸入减少了痰液的外排量。从等渗至 3% Na Cl等渗当量雾化液呈现排痰量增多、痰液干 /湿比重下降、黏度下降及中性粒细胞膜结合弹力酶表达下降的趋势 ,至 4 % Na Cl以后等渗当量时无进一步改善表现。结论 高渗雾化液较低渗雾化液更有利于 COPD患者痰液外排 ,通常最适宜渗透压条件在 3% Na Objective To explore the most suitable level of infiltration pressure in helping discharge phlegm liquid.Methods The patients with chronic obstructive pulmonary disease (COPD) were observed by inhaling atomizing liquid with gradient osmotic pressure. The phlegm liquid was collected one hour before and after atomizing inhalation, and its total amount, dry/wet weight, and the expression of membrane-bounded neutrophils elastase were observed. Results The inhalation of distilled water decreased the amount of discharged phlegm liquid. From 0.9%to 3% NaCl atomizing liquid, the patients showed the following: the amount of discharged phlegm liquid increased, the dry/wet weight decreased, the mucosity was fallen and the expression of membrane-bounded neutrophils elastase was declined. From 3% to 4% NaCl atomizing liquid, there were no further improvement in the tokens of phlegm liquid.Conclusion The atomizing liquid with hypertonic pressure is more helpful than lower osmotic pressure for sputum to be discharged for COPD patients. The most suitable level of osmotic pressure is about 3% NaCl.
