氯氮平所致的恶性综合征

CLOZAPINE—INDUCED NEUROLEPTIC MALIGNANT SYNDROME

氯氮平引起恶性综合征(NMS),国内迄今尚未见报道。本文遇到4例氯氮平治疗过程中出现高热、意识障碍伴有多汗、血压波动、心动过速等植物神经紊乱;白血球增高,1例查CPK增高,但无肌强直表现。4例脑电图均示异常。据此可能是一种非典型性或发展中的NMS表现,并建议在诊断后即刻进行治疗,以免产生不良后果。

UP to date,no case of neuroleptic malignant syndrome(NMS)induced by dozapine have been reported in domestic literature.Four cases with schizophrenia develpoing hyperthermia,confusion,diaphoresis,fluctuating blood pressure,tachycardia, leucocytosis,elevated creatine phosphokinase(CPK)activity and abnormality of EEG, but lack of muscular rigidity,during the course of clozapine treatment were presented. Because clinical manifestation don't exhibit muscle rigidity in all cases,so it can be pro- posed that this is an atypical form or a developing form of NMS in the early stage.Thus it is suggested that the treatment should be done immidiately after Diagnosis to avoid to grave consequences.
