摘要
我院1975年至1990年间手术治疗风湿性心瓣膜病700例,其中二尖瓣闭式扩张500例,瓣膜置换200例。围术期发生心室纤颤25例,抢救成活17例,死亡率为32%(8/25)。本文分析了心室纤颤的发生与疾病的严重程度、麻醉诱导不当、低血钾、术后心律和心率改变及手术创伤的关系,并对心室纤颤的急救处理及预防进行讨论。
From 1975 to 1990, 700 patients with rheumatic valvular disease were operated in our hospital. This group, consisted of 500 cases of closed mitral valvotomy and 200 of valvular replacement. VF occurred in 25 patients perioperatively with a mortality rate of 32% (8/25). Analyses of the common causes of VF indicated that the occurrence of VF was closely related to the severity of the disease, improper induction of anesthesia, hypokalemia, change of postoperative cardiac rate and rhythm and unexpected surgical trauma. Emergency management and prevention of VF were discussed.
