高血压病左室舒张功能与钙泵关系的初步探讨

MECHANISM OF IMPAIRMENT OF LEFT VENTRICULAR DIASTOLIC FUNCTION IN HYPERTENSION

为了探讨左室舒张功能损伤的细胞机制,本文测定43例高血压病患者和20例正常人的多普勒舒张功能指标和红细胞钙泵活力,43例高血压病患者又分为Ⅱ期高血压病组(n=25)和左室肥厚组(n=18)。结果显示,钙泵活力从正常组,Ⅱ期高血压病组至左室肥厚组逐渐降低;左室舒张功能从正常组、Ⅱ期高血压病组至左室肥厚组逐渐受损。63例受试者的钙泵活力与左室主动充盈指标EDC、EV/AV和EA/TA呈显著正相关(r=0.51、0.54和0.47,P<0.01)。与左室被动充盈指标AA/TA呈显著负相关(r=-0.45,P<0.01)。与等容舒张时间呈显著负相关(r=-0.50,P<0.01)。从而提示:钙泵活力与左室舒张功能关系密切,左室舒张功能减退是钙泵活力减退所致。

In order to explore the mechanism of impairment of left ventricular diastolic function, 43 patients with hypertension and 20 normal subjects were studied using Doppler left ventricular diastolic function indexes and erythrocytic Ca pump. 43 hypertensive patients were divided into stage Ⅱ hypertension(group A, n=25) and left ventficular hypertrophy (group B, n = 18) groups. The results showed: Ca pump activity of group B was significantly lower than that of group A which was again significantly lower than normals, so did left ventricular diastolic function. Ca pump activity showed good positive correlation with Doppler active filling indexes-EDC, Ev/Av and EA/TA(r = 0.51, 0.54 and 0.47, respectively, p<0.01), and good negative correlation with both Doppler passive filling index-AA/TA (r = -0.45, p<0.01) and IDT(r=-0.50, p<0.01). The results suggest that impairment of left ventricular diastolic function is induced by decrease of Ca pump activity.
