基质金属蛋白酶-2及其组织抑制物在宫颈癌中的表达

Expression of both matrix metalloproteinase-2 and its tissue inhibitor-2 in cervical carcinoma

目的 研究基质金属蛋白酶 2 (MMP 2 )及其组织抑制物 (TIMP 2 )在宫颈癌中的表达及其与宫颈癌生物学行为的关系。方法 采用免疫组化S P法对 5 7例宫颈癌组织、17例正常宫颈组织MMP 2、TIMP 2进行检测 ,并进行 χ2 检验及等级相关分析。结果 免疫组化结果显示MMP 2、TIMP 2在宫颈癌组织中表达极显著高于正常宫颈组织 (P <0 .0 1)。MMP 2、TIMP 2在宫颈癌不同组织学类型中表达无显著性差异 (P >0 .0 5 )。MMP 2表达随宫颈癌病理分级、临床分期的升高 ,淋巴结转移的发生而上调 ,但无显著性差异 (P >0 .0 5 )。TIMP 2在宫颈癌临床分期Ⅱb～Ⅳ期的阳性表达显著低于Ⅰ～Ⅱa期 (P <0 .0 5 ) ,有淋巴结转移者极显著低于无淋巴结转移者 (P <0 .0 1)。TIMP 2随病理分级升高其表达下调 ,但无显著性差异 (P >0 .0 5 )。TIMP 2与MMP 2的表达显著负相关。结论 MMP 2、TIMP 2 ,尤其是MMP 2 /TIMP 2比率失衡在判断宫颈癌侵袭性、估计其预后中有一定意义。

Objective To study the expression of matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of matrix metalloproteinase-2(TIMP-2) in the cervical carcinoma and to evaluate the relationship between the expression of both MMP-2 and TIMP-2 and biological behavior of cervical carcinoma. Methods The expression of MMP-2 and TIMP-2 were observed by immunohistochemical technique in 57 cases with cervical carcinoma and 17 cases of normal cervices. Significant differences were analyzed by χ 2 test. The correlation between levels of TIMP-2 and MMP-2 was studied with Spearman correlation coefficient. Result The positive rates of both MMP-2 and TIMP-2 are significantly higher in cervical carcinoma than in normal cervices( P <0.01).There is no statistical difference among these cervical carcinomas of different histological types( P >0.05). The expression of MMP-2 is no relevant to distinct clinical stage,pathologic grade and lymph node metastasis( P >0.05).The expression of TIMP-2 decreases significantly with the rise clinical stage and lymph node metastasis( P <0.05),but had no relevant to distinct pathologic grade( P >0.05).There is a very significant correlation between the expression of TIMP-2 and MMP-2( P <0.01). Conclusion Both MMP-2 and TIMP-2, especially the unbalance in MMP-2/TIMP-2 have probaly a prognostic value in cervical carcinoma.
