奥沙利铂加氟脲嘧啶持续静脉滴注治疗晚期食道癌

Efficacy and the toxicity of 5-fluououracil continous infusion combined with leucovorin and oxalipatin in the treatment of advanced esophageal cancer

目的 观察奥沙利铂联合醛氢叶酸和氟脲嘧啶持续静脉滴注治疗晚期食道癌的疗效及不良反应。方法 经病理组织学诊断的晚期食道癌 35例 ,奥沙利铂 (Oxaliplatin ,L -OHP ,法国赛诺菲公司生产 ,商品名为乐沙定Eloxatin) 130mg/m2 静脉滴注 2h ,第 1天 ;醛氢叶酸 (LV) 2 0 0mg/m2 ,静脉滴注 2h ,随后氟脲嘧啶 (5 -Fu) 375mg/m2 静脉推注 10min ,再接 5 -Fu 3 0g/m2 用输液泵连续滴注 12 0h ,2 1天为 1周期。所有患者至少接受 2个周期的治疗。结果 35例均可评价疗效 ,总有效率 5 7 1%。初治病例 2 2例 ,有效率 6 3 6 %。复治病例 13例 ,有效率 4 6 2 %。主要不良反应为神经毒性 ,骨髓抑制 ,恶心 /呕吐 ,腹泻 ,粘膜炎等。无治疗相关死亡者。结论 L -OHP联合LV和 5 -Fu持续滴注治疗晚期食道癌患者 ,缓解率较高 ,毒性相对较低 。

Objective To observe the efficacy and toxicity of oxaliplatin combined with leucovorin/5-fluorouracil continuous infusion in the patients with advanced esophageal cancer.Methods thirty five patients with a histologic diagnosis of advanced esophageal cancer were obsered.The chemotherapy regimen was comprised of L-OHP 130mg/m 2 infusion for 2 hours,LV 200mg/m 2 infusion for 2 hours followed by 10 minute intravenous bolus of 5-Fu 375mg/m 2,then 120 hours infusion of 5-Fu3.0/m 2 with an ambulatory pump,repeated every 3 weeks.All patients were received at least two cycles of the treatment.Results Twenty three patients were enrolled in this study.The total response rate was 57.1%.The RR of 22 the first treatment patients were 63.6%.The RR of 13 recurrent patients were 46.2%.The major toxicity included nerve toxicity,neutropenia,nausea vomiting and mucositis.No treatment-related death occurred.Conclusion Oxaliplatin combined with leucovorin/5-fluorouracil continuous infusion for treatment of advanced esophageal cancer had moderate activity and comparatively mild toxicity.Further clinical investigation should be warranted.
