丙泊酚及咪唑安定单独或联合应用时脑电双频指数与血液动力学变化的相关性研究

摘要 目的 研究丙泊酚、咪唑安定、丙泊酚与咪唑安定联合应用时BIS的变化及其与血液动力学变化的相关性。方法 选择45例择期拟行全身麻醉、非颅脑手术患者随机分为三组:Ⅰ组(15例)静注丙泊酚1mg/kg;Ⅱ组(15例)静注咪唑安定0.2mg/kg;Ⅲ组(15例)静注丙泊酚1mg/kg+咪唑安定0.1mg/kg。随后均用芬太尼2μg/kg、维库溴铵0.15mg/kg静注,气管内插管。记录麻醉诱导前、诱导中、插管前及插管后的HR、BP、SpO_2,观察整个诱导期BIS的变化及其与血液动力学变化的相关性。结果 三组患者用药后BIS进行性降低至30以下,而插管后则有上升趋势。BIS变化与血液动力学变化明显相关(r>0.6)。结论 丙泊酚、咪唑安定以及二者联合应用时BIS进行性降低,与血液动力学变化有相关性。 Objective To study the BIS changes and relationship between BIS and hemodynamics during anesthesia induction with propofol and/or midazolam. Methods 45 patients undergoing different operations except head procedure were divided into three groups randomly with 15 cases each. Propofol 2 mg/kg was given in group Ⅰ,midazolam 0.2 mg/kg in group Ⅱ,and propofol 1 mg/kg plus midazolam 0.1 mg/kg in group Ⅲ,which was followed by fentany 2μg/kg,vecuronium 0.15 mg/kg, and then the trachea was intubated. HR, BP and SpO_2 were recorded before drug administration, at 1 min after that,before and 5 min after intubation. Results The BIS of three groups decreased continuously after induction and increased after intubation. BIS and hemodynamic changes were possitively correlated with the coefficient of greater than 0.6. Conclusion Propofol and/or midozolam can make BIS decreased, which is correlated positively with the hemodynamic changes.
