检测尿脱落细胞中CK-20表达的临床意义 被引量:1

摘要 目的 探讨膀胱癌病人尿脱落细胞CK-20的表达情况及其可能的临床意义。方法 尿液标本经适当处理后,采用RT-QCR方法检测尿脱落细胞中CK-20 mRNA的表达情况,分析其与临床病理参数的关系,了解其能否为膀胱癌的一种无创检测手段。结果CK-20 mRNA表达在膀胱癌病人尿中阻性率为40.48%(17/42),在浸润性肿瘤病人尿中阳性率较高,但元统计学意义(P>0.05);在复发病例中阳性率(60%)比初发者中(22.73%)明显要高(P<0.05)。但在所有正常对照者尿中均无表达。结论检测尿脱落细胞CK-20表达可以作为膀胱癌的一种无创诊断及筛查手段。 Objective To investigate the expression of cytokeratin - 20 mRNA and it's potential clinical implications in urinary cytology of patients with bladder carcinoma. Methods After treating with proper method, urine specimens from 69 individuals were analyzed by RT - PCR for the expression of CK- 20 mRNA and it's relation to the clinico-pathologic parameters. Results The overall sensitvity of CK -20 gene expression in voided urine samples with bladder neoplasms was 40.48% (17/42) . It is relatively higher in the tumor of advanced stage and grade, but there isn't significance ( P > 0.05) . It's significantly higher in recurrent cases(60%, 12/20) than that in primary cases (22.73%,5/22)(P<0.05). But CK-20 mRNA wasn't expressed in the control group. Conclusion CK - 20 is a potential bio-marker for noninvasive detection of bladder carcinoma by assaying exfoliated cells from the voided urine specimen with RT-PCR.
