广西肺结核高治愈率和高发现率策略研究

Studies on the strategies of improving cure and detection rate of patients with pulmonary tuberculosis in Guangxi

目的研究影响广西结核病高发现率和高治愈率的因素 ,寻求提高两率的措施。方法从广西历次结核病流调的疫情升降趋势和传染性肺结核病人治疗的队列分析进行研究。结果涂阳病人的治愈率54.2 %,完成疗程率16.9 %,失败率5.9 %,丢失率14.4 %。目前发现1例涂阳病例就有4例涂阳病人漏报漏登 (或为假阴性而漏掉 ) ,还有2例没有发现。结论加强结核病疗程结束时的查痰以及加强病人管理防止过多丢失是提高治愈率的两项最简单有效的措施 。

Objective To explore the strategies of improving detection and cure rate of patients with pulmonary tuberculosis in Guangxi. Methods The epidemic trend of epidemiological sampling surveys from tuberculosis for the past four times and the data of cohort analysis with current infectious tuberculosis in Guangxi were analysed. Results It was showed that The cure rate of the sputum smear positive pulmonary tuberculosis cases was 54.2%,completed treatment rate 16.9%, failure rate5.9%and defaulted rate 14.4%.It was found that among 7 sputum smear positive tuberculosis cases ,one of them was reported only, four cases were neglected and two cases were not found. Conclusion More sputum smear examination, especially at the end of medication, and more effective administration to prevent from defaulting are the approaches of improving the cure rate of patients with tuberculosis.and improving convergence case_mangement of TB suspects is a effective method to improve detection rate.
