颈椎病的临床X线分型

Clinical X-ray typing of the cervical spine disorder

目的探讨颈椎病的临床X线分型。方法着重对比分析576例患者的临床症状与X线分型关系。结果研究发现临床症状与X线表现相关有4个方面:a)椎体间关系改变,生理曲度变直、后弓、顺列失常、椎体不稳、滑脱,旋转、可牵拉推移压迫血管神经。b)椎体骨质增生特别是后缘增生可直接压迫血管神经。c)椎体小关节突骨质增生致椎间孔变小往往可直接导致相关血管神经受压。d)韧带钙化或骨化可导致活动受限,以上4个方面因素足可以对椎体周围血管神经肌肉产生压迫推移、牵拉或刺激而引起一系列临床症状。结论根据临床表现结合X线改变提出颈椎病临床X线分型意见。颈椎病是由于椎体及其附属结构退行性变引起的一系列临床症侯群。

Objective To study the clinical X-ray typing of the cervical spine disorder.Methods The relationship between clinical symptoms and X-ray findings collected from 576 patients with cervical spine disorder were investigated retrospectively and then the clinical X-ray typing of this disease was recommended.Results The study showed that there were relationships of clinical symptoms and X-ray findings in four aspects,i.e.a)interpyramidal relation change such as physiological curve loss,backward arching,alignment abnormality,vertebral body unsteadiness,dislocation,rotation etc.these changes could cause the traction,push and pressure ofc vessels and nerves.b)bony proliferation of vertebral body,in especial,posterior borders proliferation could directly oppress the vessels and nerves.c)bony proliferation of small joint process could cause the narrowing of intervertebral foramina and frequently could oppress the neighbour vessels and nerves.d)calcification and ossification of ligamenta could cause the limitation of movement.Conclusion Based on the clinical manifestations and X-ray findings of cervical spine disorder,a recommendation relating to X-ray typing of this disease is proposed in the article.Cervical spine disorder is a serial clinical symptoms induced by the degenerative change of vertebral body and accessory structures.
