不同白内障手术切口对兔角膜内皮细胞的影响 被引量:2

摘要 目的 比较不同白内障手术切口对兔角膜内皮细胞密度的影响。探讨手术切口损伤角膜内皮的机制 ,为术中减少角膜内皮损伤创造有利条件。方法 将 60只兔眼随机分为三组 :A组 (小切口组 )、B组 (中切口组 )、C组 (大切口组 ) ,每组 2 0只眼。分别于术前 ,术后 3、14、3 0d观察兔角膜上方和中央区内皮细胞密度的改变 ,计算内皮细胞丢失率 ,进行统计分析 ,比较各组内皮细胞丢失率之间的差异。结果 随手术切口的缩小 ,上方角膜内皮细胞丢失率呈下降趋势 ,角膜中央区的内皮细胞丢失率随上方角膜内皮细胞丢失率的下降而下降。A组与B组术后各期内皮细胞丢失率无差异 ,而A组与C组、B组与C组之间内皮细胞丢失率存在显著性差异。结论 白内障手术切口越小对角膜内皮细胞的保护越好 ,自闭性小切口应是临床白内障手术的理想切口。 ObjectiveTo compare the influence of different incisions on the rabbit corneal endothelial cell density in cataract surgery.MethodsSixty rabbit eyes were randomly divided into group A (3.2 mm incision),group B (6 mm incision) and group C (>10.0 mm incision).Changes of the rabbit corneal endothelial cell density in the superior and central area were observed,and the rate of endothelial cell loss was calculated.ResultsWith the contraction of operative incision,the loss rates of superior corneal endothelial cell tended to reducing,and the reduction of loss rate in central endothelial cell was consistent with that of superior endothelial cell.There was no statistical difference between the group A and group B at each period,but greater difference between group B and group C,group A and group C.ConclusionsThe size of operative incision can affect corneal endothelium directly; auto-closed small incision is the ideal incision for cataract extraction.
