动静结合治疗骨折的实验研究——循环应力对骨折愈合早期的影响 被引量:8

摘要 "动静结合"治疗骨折是祖国医学的正骨原则,认为良好的固定应与适时的功能锻炼相结合。肌肉的收缩和关节的屈伸运动可使骨折端产生轴向循环应力。本文用自制弹簧外固定支架及骨折循环应力治疗仪对家兔桡骨骨折进行实验研究。实验组在骨折端用弹簧外固定支架加50牛顿恒压,并用循环应力治疗仪施以频率1赫兹,峰值6.7牛顿的循环应力每天1小时;对照组单纯用弹簧外固定支架。通过对骨折愈合早期(5~18天)骨痂的生物力学测定及组织学观察,结果发现:循环应力能刺激骨痂大量生成。促进骨痂成熟,从而提高骨的材料特性和结构特性,达到加速骨折愈合的作用。 The theory of combining rest with motion is a principle of bone-setting of Traditional Chi- nese Medicine,which requires a reliable fixation and some associated runctional trainings in proper times dur- ing the treatments of fractures.The contractions of muscles and the extantions of joints may produce circle- stress between the fractured ends longtudinally.The authors have documented the utility of the self-made ex- ternal spring fixator and the circle-stress producer for bone—healing in the experimental fractured bones in rabbits.A uniform pressure of 50 newtons was applied on the fractured bones by the fixator as well as a cir- cle-stress with a frequency of 1 HZ and a maximum pressure of 6.7 newtons by the circle-stress producer in the experimental group.The control group was treated only by the external spring fixator.Biomechanical measurements and histological changes of the callus were observed in the early period of bone-healing(5-18 days).The results demonstrate that circle-stress can greatly stimulate the productions of callus and the matu- rity of it.Thus,the material behaviors and the structural behaviors of callus were imoroved and the effect of expediting bone-healing was achieved.
