逆行交锁髓内钉与钢板治疗股骨髁部骨折的疗效比较

Comparison of the clinical of retrograde interlocking intramedullary nail and plate in treating femoral intercondylar fracture

目的 :评价逆行交锁髓内钉与钢板治疗股骨髁部骨折的疗效。方法 :回顾分析应用逆行交锁髓内钉治疗股骨髁部骨折 3 1例 ,钢板治疗股骨髁部骨折 45例的恢复情况。结果 :经过 6～ 18个月的随访 ,逆行交锁髓内钉治疗组骨折愈合率 10 0 %,骨折平均愈合时间 4个月 ,感染率为 3 .2 %。钢板治疗组骨折愈合率 91.1%,感染率 11.1%,2例钢板外露 ,骨折平均愈合时间 5 .5个月。结论 :逆行交锁髓内钉治疗股骨髁部骨折 ,操作简单 ,固定坚强 ,对骨折局部血供破坏小 ,是一种疗效满意的方法。治疗结果优于钢板 ,但其应用有一定局限性。

Objective:To evaluate the efficacy of retrograde intramedullary nail and plate in the treatment of femoral intercondylar fracture.Method:A retrospective analysis was done on the recovery of 45 cases with femoral fractures with the retrograde intramedullary nail and 31 cases treated with plate.Result:The 6～18 months' follow-up showed that the healing rate and the infection rate were 100% and 3.2%,respectively in the Group A with a mean healing time of 4 months and 91.1% and 11.1% respectively in the group B with a mean healing time of 5.5 months and 2 cases plate exposed. Conclusion:The retrograde intarmedullary nail has reliable fixation,simple operation.Less trauma on regional blood supply and is more effective in the treatment of femoral intercondylar fracture than the plate.Nevertheless,The retrograde intramedullary nail itself has limitation for further application.
